NOMINATION FORM

The Netball ScotlandService Award provides the opportunity for theGoverning Body and the Scottish netball community to recognise and honour those individuals who have rendered outstanding voluntary service to netball at any level of the sport over a long period of time (measured in years’ service to sport).

Nominations for this Award can be made at any time of the year and will be recognised at the nearest Awards Dinner in that calendar year.

Criteria and Process for Nomination

To qualify for recognition at the Awards Dinner within a calendar year, nominations can be submitted at any time up to at least 12 weeks prior to the Netball Scotland Awards Dinner (the dinner takes place by mid-November each year).

The Board or any member organisation (school, club, District, Region) may submit nominations. A Netball Scotland Service Award may be granted to any person who has rendered outstanding voluntary service to Netball Scotland at any level of the sport over a long period of time (measured in years’ service to the sport).

The Board shall appoint a Service Awards Panel consisting of no less than three (3) members, one of whom will be a member of the NS Board who will chair the panel. If in the opinion of the Panel, the nomination meets the criteria, successful nominations shall be put forward to the Board for endorsement.

Nominations for the 2015 Awards Dinner should be received beforeWednesday 30thSeptember 2015. Any nominations received after that time will not be included.

Awards will be presented at the 5th Annual Netball Scotland Awards Presentation at the Hilton Hotel, Grosvenor on Saturday 14thNovember 2015.

When completing the form, focus on the key accomplishments and how they have impacted on people’s lives. Ensure that you have recorded specific dates and that it is verified by at least two members of your school, club, District or Region.

The Nominee mustbe a member of Netball Scotland.
