This induction checklist should be completed for ALL users of the CRF, such as Principal and Co-Investigators, research nurses, trial co-ordinators, radiographers and scientists (list not exhaustive).

Completion Guidelines:
This document must be completed in conjunction with a CRF manager and/or a member of the CRF’s clinical team.
On completion, it must be signed/dated by the CRF user and the trainer, uploaded to Q-Pulse and filed in the CRF Users’ Induction Files in the QA Manager’s office.
Training in specific areas may be given by a CRF manager or other nominated trainer.
If there are points in the checklist that are not applicable to the new starter, the trainer must enter N/A in the ‘Applies?’ column.
