Credentialing Requirements:
Each Radiation Oncologist and Physicist team must complete the credentialing process before a patient can be placed on the protocol (See Section 5.1 of the protocol). Once the team has met the minimum requirements for credentialing, a letter will be sent to the Radiation Oncologist from NSABP informing them that they have successfully completed the credentialing process and can begin placing patients on the study.
- Each Radiation Oncologist and Physicist team at an institution must complete the PBI QA Knowledge Assessment and Facility Questionnaires and complete the benchmark case for each PBI technique for which the institution wants to be credentialed. (Note: If a Radiation Oncologist at the same institution has been credentialed previously, then all subsequent Radiation Oncologists need ONLY to complete the PBI QA Knowledge Assessment Questionnaire and Sections I and II of the PBI Facility Questionnaire.)

Benchmark Cases: For each PBI technique (3D CRT, MammoSite or Multi-Catheter) for which an institution would like to be credentialed, the specific benchmark case must be planned per protocol and submitted electronically to the ITC (see below on How to Submit Digital Data). A completed PBI data form (either 3D CRT or MammoSite/Multi-Catheter) must be completed and a hard copy of the plan, including isodose lines, must also be mailed to IROC Houston at: 8060 El Rio, Houston, TX 77054

Philips Pinnacle users:
Below are the 3 PBI benchmark cases, which have been saved as UNIX files. This enables all Pinnacle users to import the CT image sets into their treatment planning systems. These data sets can be restored as is on a Pinnacle workstation by using the Pinnacle Launch Pad Restore utility. No physics data are included with this archive so it can be restored into an existing institution. The user will need to select his or her own CT table when first opening the plan.
Multi-Catheter (52 CT images.) The GTV can be found on slices 150.43 cm through 153.13 cm.

CMS XiO users:
DISCLAIMER: these patients are provided as a convenience to XiO users for the sole purposes of obtaining credentials for the Breast protocol. CMS makes no claims as to the accuracy of the image or structure data – neither spatial nor density information. These CT data sets are compatible with XiO 4.2.0 and later.
Below are the 3 PBI benchmark cases, which have been saved as tar files. Each tar file contains one XiO patient. The patients have been stored at the XiO patient directory level using XiO 4.2.0 and should be “untarred” in this directory (instructions below).
To unload from the CD,
Log on as root
Type /Focus/bin/mount_cdrom
cd to the XiO clinic in which you wish to develop the plans (e.g. cd /Focus/rtp1/1)
cd patient (this puts you in the patient directory)
cp /cdrom/XiO*.tar . (copies the tar files from the CD to your disk)
ls –la *.tar (confirm the load from CD; lists the tar files in the patient directory)
If you see the 3 tar files, unmount the CD as follows….
/Focus/bin/unmount_cdrom
eject
To unload the patients into the clinic…
Tar –xvf XiORPC3DCRT.tar (untars the first patient, RPC1)
Repeat this last line as needed for the other tar file names (XiORPCMammosite.tar or XiORPCMulticath.tar)
Log out of root and check that you can open each patient inside XiO.
