CRANES AND RIGGING — INTRODUCTION/PREUSE

CRANE OPERATOR TRAINING AND LICENSING

1.0  GENERAL

Note: In the United States (U.S.) the following requirements/options are applicable until October31, 2014, at which time these requirements become null and void.

Note: Certain U.S. states (such as Washington) and municipalities or similar (such as New York City) have already enacted regulations on this subject — full compliance with such regulations is mandatory; such regulations supersede all of the following requirements/options.

Crane operator training and licensing (called crane operator qualification and certification by some U.S. and non-U.S. regulatory agencies) is a P2S requirement for both self-perform and contractor/sub-/lower-tier contractor crane operators.

1.1  Training/Licensing – Certification Options

Training and licensing/certification can be accomplished in 2 basic ways:

·  By an internal/P2S-managed process

·  By an external non-P2S-managed process

If done through an internal/P2S-managed process, the training and licensing/certification—while transferable from project-to-project—is not transferable to an employer/entity other than P2S.

1.2  Recognizing Third Party Training/Certification

It is recommended that training and licenses/certifications from a “recognized” external source (such as NCCCO, NCCER, union-sponsored apprenticeship programs, the U.S. military, or a governmental entity) be accepted “as-is”, without further formal training or testing. HOWEVER, this approach is a project-specific decision, to be made concurrently by the project/construction manager and the Business Unit HSE lead.

1.3  Ensuring Proficiency

Whatever the decision regarding additional training and/or testing, it is still the project’s responsibility to ensure (to the extent feasible) that every crane operator is sufficiently proficient to operate every crane to which he/she is assigned. Also, refer to Section 5.0 of this attachment.

1.4  Decision-Making and Documentation

Each project is responsible for determining the method of training, licensing/certification, and ensuring proficiency, as well as developing and keeping all associated documentation.

2.0  INTERNAL/P2S-MANAGED PROCESS

To be eligible for licensing by P2S as a crane operator, the trainee or applicant must meet the following prerequisites:

·  Graduated from a recognized crane operator training program, such as (but not limited to):

-  North American Crane Bureau

-  National Commission for the Certification of Crane Operators (NCCCO)

-  National Center for Construction Education and Research (NCCER)

-  P2S-sponsored

-  U.S. Military (any branch)

-  U.S. state or locality-sponsored program

-  Union-sponsored apprenticeship program that is nationally, state, or incountry approved

Note:  A signed/dated certificate of graduation on training program “letterhead” must be presented by the applicant.

·  A minimum of 6months of experience in oiling or equivalent work experience on cranes.

·  Successfully pass adrug screen. Projects that do not have adrug screening program must contract with aphysician, who properly collects the specimen and forwards it to a “recognized” (preferably certified) laboratory for processing/testing.

Note:  Refer to Practice 000.653.1400, Substance Abuse Prevention Program; and U.S. Human Resources Policies HR144, Substance Abuse Prevention and Chemical Screening Program; and HR505, Substance Abuse Screening (Field Project Sites).

Note:  If a trainee/applicant fails a drug screen, no further processing is allowed; the employee is managed according to drug screen/substance abuse procedures.

·  Successfully pass a physical examination performed by alicensed healthcare professional (LHCP) conducted in accordance with Practices 000.653.2100, PreProject Medical Examination — International, or 000.653.2104, PostEmployment Job-Related Questionnaire — U.S. (as applicable)

The examination includes, but is not limited to, eye examination (depth perception, visual acuity, and peripheral vision), blood pressure, and past health history. An EKG, chest xray, and audiometric test are performed at the LHCP’s discretion.

If the physical is performed by a nurse or EMT and a discrepancy or abnormality is found, the applicant must be referred to aphysician.

Upon completion of the physical examination, the results of the examination are forwarded to the project.

·  Successfully pass a general knowledge written examination (80percent is “passing”).

·  Successfully pass exams on “hand signals” and “load chart” with a 100percent score.

·  If the trainee/applicant is to be licensed for a lattice, overhead, tower, or ringer/transilift crane, or air tugger, successfully pass awritten examination specific to each type of crane to be operated.

·  Successfully pass an onmachine operator skills test for each type of crane to be operated (types of cranes are listed on page 4 and 5 of this attachment).

·  Successfully pass a performance evaluation given after at least 30 days of operating a crane on the job.

·  Attend and successfully complete a crane operator refresher training program every 2years.

Crane operators (and licensors) will be licensed to operate cranes based on the types of cranes outlined in Section3.2.

3.0  LICENSOR

3.1  Licensor Prerequisite Requirements

The licensor must meet the following requirements:

·  Five years experience in operating hydraulic or friction cranes.

·  Sixmonths continuous service with P2S or the subcontractor/lower-tier contractor (asapplicable).

·  Is currently “licensed” in accordance with this attachment.

·  Is currently foreman level or above and has held that position for 30days or more.

·  Is recommended by project or group management.

·  Has completed the following (provided by a recognized supplier such as (but not limited to):

-  North American Crane Bureau

-  National Commission for the Certification of Crane Operators (NCCCO)

-  National Center for Construction Education and Research (NCCER)

-  P2S-sponsored

-  Union-sponsored apprenticeship program that is nationally, state, or incountry approved

-  U.S. Military (any branch)

-  U.S. state or locality-sponsored program

-  Crane licensor safety orientation program

-  Ageneral crane safety knowledge written examination consisting of (at least, but not limited to) the use of approved hand signals, general operational characteristics, crane safety procedures, and capacity chart calculations

-  An “onmachine” crane operators’ skills evaluation test at time of licensor qualification

-  An annual crane operator refresher training program

·  Is knowledgeable of Practices 000.653.3201, 000.502.0100, and 000.502.1000, safe crane operation procedures; current OSHA, ASME, and other applicable crane-related operator requirements; and the responsibilities of crane operators and crane operator licensors.

3.2  Types of Cranes

In order to issue a license, the licensor must be authorized in writing by the Business Unite HSE lead for each type of crane. This authorization is granted to the licensor based on his/her knowledge and experience of the following types of cranes:

Mobile Cranes

·  Lattice Boom

-  Crawler

-  Rubber tired

·  Telescopic Boom (Hydraulic)

-  Fixed cab (0 –22 ton)

-  Rotating cab (22 – 65 ton)

-  Rotating cab (> 65 ton)

·  Boom Truck

Tower Cranes

·  Fixed jib

·  Luffing jib

Bridge/Overhead Cranes (includes Polar)

·  Cab Controlled

·  Floor Controlled

Drum Hoist

Ringer/Transi-Lift

Air Tugger

3.3  Licensor Recertification

Each licensor must be recertified according to the above requirements every 4years, within 30days of their previous certification date.

3.4  Licensor Responsibilities

Licensors are responsible for performing the following:

·  Onmachine operator skills tests

·  Coordination of the 30day performance evaluation with trainee/applicant’s immediate supervisor

·  Licensing crane operators based upon completion of required training and successfully passing the written tests, skills test(s), performance evaluation, physical examination, and drug screen.

4.0  TRAINING

A crane operator must demonstrate the following:

·  Have a complete knowledge of basic machine operation, including equipment capabilities and lever and pedal functions.

·  Understand hand signals associated with crane operations.

·  Understand basic engineering principles of equipment such as levers, gears, shafts, or chain drivers.

·  Understand the operation of the crane and being able to travel the crane.

·  Understand, read, and interpret load charts.

·  Ability to calculate exact load sizes and weights.

·  Understand the proper methods of maintenance, care, and inspection of cables/lines.

·  Understand the vocabulary used in crane operation

·  Thorough knowledge of the machine’s maintenance and lubrication requirements.

·  Understand the methods of time and equipment reporting.

·  Ability to complete a comprehensive inspection and be able to document results in an understandable manner.

Trainees/applicants who successfully complete training and skills testing undergo a30day “evaluation period,” followed by a performance evaluation.

5.0  SKILLS TESTING/PERFORMANCE EVALUATION

Note:  This section (all or any part) may also apply to operators licensed/certified by a recognized external (to P2S) entity, at the discretion of the Business Unit HSE lead.

The skills test and 30day performance evaluation are given to measure the trainee/applicant’s operational skills under varying conditions for each specific model of crane for which he/she is to be licensed.

The skills test is given after training has been completed, and covers the responsibilities for inspection (prestart and poststart) and preventive maintenance, as well as the crane’s operational characteristics, load/capacity chart comprehension, standard hand signal recognition, use and demonstration of operator aids, and safety shutdown procedures for the make and model of the crane on which the test is being conducted.

Performance evaluations are essentially the same as the “skills test,” but are performed within 30days of an on-the-job “evaluation period” (or for previously trained applicants, after initial hire/assignment) and prior to issuing an Equipment Operator’s License (Form 000.653.F0126, available only from Greenville Forms Management).

The Crane operator skills test and performance evaluation forms are:

·  Crane Operator Skills Test/Performance Evaluation (Tower), Form 000.653.F0298

·  Crane Operator Skills Test/Performance Evaluation (Bridge/Overhead), Form 000.653.F0299

·  Crane Operator Skills Test/Performance Evaluation (Drum Hoist), Form 000.653.F0300

The skills test and performance evaluation forms provide a list of activities/tasks that should be used to evaluate the skills of the operator. Additionally, the licensor must evaluate the trainee/applicant’s ability to understand and properly use the load/capacity charts and any operator aids applicable to the crane on which testing is being conducted.

If, at any time during the skills test or performance evaluation, the licensor determines that the trainee/applicant shows an obvious lack of operating skills, does not perform up to expectations, or for some other reason does not pass the skills test, the licensor (at his/her discretion) must discontinue testing and note the justification for such on the test form.

The licensor will maintain and distribute copies of skills tests and performance evaluations conducted whether the operator passes or fails.

6.0  LICENSING

Note: This section (all or any part) may also apply to operators licensed/certified by a recognized external (to P2S) entity, at the discretion of the Business Unit HSE lead.

Upon successful completion of the training course(s), written examination, skills test, 30day evaluation period, and performance evaluation, a license is issued to the trainee/applicant by the licensor for a period not to exceed 24months. The license lists the make(s), model(s), and type(s) of cranes the operator is authorized to operate and any restrictions.

A crane operator’s license from previous jobs must be verified from (P2S) Business Unit to Business Unit.

The license should be transferable between projects, subject to the Project/Site Manager’s and Business Unit HSE lead’s concurrent approval.

Trainee/applicants failing any element of training/testing will not be hired as crane operators, and are not eligible for retesting until they (again) receive and pass the training course(s).

7.0  RECORD KEEPING

ACrane Operator License Record (Form 000.653.F0127) will be completed by the project for each crane operator after successful completion of the licensing requirements.

The original will be retained by the project and acopy distributed as deemed appropriate by the project. (Subcontractors maintain documents on site.)

Note:  A crane operator license is valid 24months from month of issue. Anew Crane Operator License Record must be completed each time anew license or change is to be recorded. The appropriate box on the Crane Operator License Record must be checked to indicate anew license, arenewal, asuspension, arevocation, or areinstatement transaction.

8.0  LICENSE RENEWAL

Every 12months, licensed crane operators must pass aphysical exam and adrug screen test. Every 24months the licensed crane operator must successfully complete acrane operator refresher training program, awritten examination, and aperformance evaluation for each type of crane for which the license is to be renewed. The physical examination, the written examination results, and the performance evaluation results will be documented and retained by the project.

The operator should initiate the license renewal process within 30-60days of the license expiration date.

The licensor completes the appropriate skills test/performance evaluation form and the Crane Operator License Record, indicating arenewal transaction.

9.0  SUSPENDING THE LICENSE

Licenses are automatically suspended, and the operator must not operate any crane/air tugger, when:

·  The operator’s physical examination becomes delinquent (366days from date of last examination) or if the operator fails to pass the physical examination. Upon passing the physical examination and drug screen test, the license is reinstated.

·  The operator’s annual refresher training becomes delinquent.

·  An accident has occurred.

Note:  The license will be revoked if the operator is found negligent, or will be reinstated without prejudice based on the facts.

·  The operator fails adrug screen.

The Suspension box is checked on the Crane Operator License Record to suspend a crane operator’s license, and the reason for suspension is noted in the Comment section of the form. Written justification for suspension will be kept in the operator’s personnel file. Copies of the Crane Operator License Record are distributed as noted in Section7.0. The Project/Construction Manager retrieves the license from the operator when the license has been suspended.

10.0  REVOKING THE LICENSE

A license will be revoked by the licensor or Project/Construction Manager for a period of 6months (or more, at their discretion) for reasons such as the following:

·  Inability to pass the physical exam

·  Unsatisfactory performance

·  An accident due to operator negligence

·  Safety violations involving hazardous equipment operations

·  Falsification of licensing records

·  Positive results of a drug screen

Further disciplinary action will be carried out in accordance with existing disciplinary practices.

The “Revocation” box is checked on the Crane Operator License Record to revoke an operator’s license, and the reason for revocation will be noted in the “Comment” section of the form. Written documentation supporting justification of this action will be kept in the operator’s personnel file. Copies of the Crane Operator License Record are distributed in accordance with Section7.0. The Project/Construction Manager is responsible for retrieving the crane operator’s license when the equipment license has been revoked.
