Targeted Audience: All staff and clinicians

Purpose:

  • Communicate the importance/goals of the CQN Project
  • Lay foundation for decisions to be made regarding current ACIP Recommended and Catch-Up Schedulesfor Children and Adolescents Aged 18 Years or Younger

CQN Practice Key Driver #1: Physician Lead Champion Responsible for Driving Improvement

Intervention #2: Formally communicate to the practice the importance and goal of this project

Intervention #3: Ensure providers agree to follow ACIP Recommended ImmunizationSchedule/Catch-Up Schedule

Suggested Meeting Length:

Overview of CQN Project: 25 minutes

Overview of Project with discussion of ACIP Recommended Immunization and Catch-Up Schedules: 40 minutes

Meeting Preparation:

Review and add content to the slides, as requested below, that are specific to your practice. Have computer, screen and slide deck loaded prior to the meeting. Physician leader will want to review slides and notes at bottom of the slides for key messages/ideas

Materials: Suggest bringing the Practice Immunization Playbook to reference during the presentation, to access or reference materials as discussion/questions arise.

  • Slides 14-22; Populate slides 20: Baseline Data, Slide 21: QI team/roles; Slide 22: Testing/Areas of focus

  • Optional: Cover content if clinicians need information on MOC
  • Slides 23-26

Part 5: ACIP Recommended Immunization and Catch-Up Schedules
5-15 minutes /
  • Overview of the ACIP Recommended Immunization Schedule and Catch-Up Schedule
