REIMBURSEMENT

June 6 - 10, 2016

Please Note:

·  For international attendee reimbursements if you have a B-1, B-2, WB, WT, F-1- G4, H1-B, O1 visa status, please complete the expense reimbursement form and W-8BEN tax form both of which are in your packet and also located on the Cowles website at http://cowles.econ.yale.edu/

·  We will need all original receipts, boarding passes (arrival and departure), itinerary and payment for travel reimbursement. Receipts must be received no later than Tuesday, September 6, 2016. You can send them directly to me at the address listed below:
