Important Note:

  1. This form should be submitted by outbound exchange studentat least 1 month before the start of exchange study.
  2. ListALL of the courses you plan to take at host institution and the equivalent CityU courses/degree requirements for credit transfer, if any. Consult your Academic Advisor on the course plan. You are also advised to consult the Exchange Coordinator (Dr. Patrick Sit) on the suitability of identified courses, if necessary.
  3. A list of pre-approved courses at different host universities can be foundon SEE outbound exchange webpage. For courses that have not been pre-approved for credit transfer, please attach the course information including topics covered, assessment methods, recommended textbook title(s) and pre-requisites, total hours of student work, etc. In addition, if you wish to transfer credits towards GE Requirements when an equivalentGE courses cannot be identified, please attach the GE Course Credit Transfer Pre-approval Form from Office of Education Development and Gateway Education as well.
  4. You will be notified of the outcome via email after your application is reviewed.
  5. If you change the list of courses as indicated in this form after arriving at host institution, it is your responsibility to notify the Exchange Coordinator via SEE General Office (Ms. Debbie Leung– ) and submit an application again for any newly-added courses that you wish to transfer credits. Your request will be reviewed which may take 1-2 weeks, please make sure you leave enough time should you need to meet the add/drop period of host institution.
  6. You are required to submit an official transcript of the exchange study to SEE General Office by the first week of the semester following the exchange study, or as soon as you receive it, whichever is sooner. The credit transfer will only be confirmed after the review of the transcript.
