Title: Case Manager

Supervisor: Director of Social Services

ROLE: The Case Manager will provide support and care coordination to adults age 65+ who are recent or long term survivors of trauma resulting from abuse. The Case Manager will provide information and help clients access appropriate services and develop skills needed for safety planning and resiliency. The position is based at the Council on Aging office and works in coordination with other Council on Aging services, in addition to working closely and exchanging referrals with elder law attorneys, Adult Protective Services and other community resources.

RESPONSIBILITIES:

·  Adherence to Council on Aging policies, procedures and core values.

·  With direction from supervisor, implement grant requirements with fidelity.

·  Ability to work independently, with sound judgement and professional demeanor.

·  Assess client needs and identify appropriate trauma informed, client centered services.

·  Establish care plans with particular attention to client safety and appropriate living situations, navigation of justice system and/or interaction with law enforcement, crisis intervention, and stabilization. Case management may be short or long term.

·  Conduct home visits and/or meet with clients in agreed upon safe locations.

·  Work with a diverse population that includes clients who may be impoverished, have physical or mental challenges, drug and/or alcohol dependencies, complicated family dynamics, and varied cultural backgrounds.

·  Strive to educate and empower clients, and be persistent in helping them achieve mutually agreed upon goals.

·  Attend relevant trainings and meetings to expand areas of expertise and knowledge of community resources.

·  Complete required reports on time and maintain records in an orderly manner, according to established protocol.

QUALIFICATIONS:

·  Education and/or training in issues related to victimization of older adults, including domestic violence, sexual abuse and financial abuse. Experience in trauma-informed care, crisis intervention, suicide prevention and case management.

·  Master’s Degree in social work or psychology preferred.

·  Bachelor’s Degree in a relevant field, with 5 years of experience working with older adults in a social services setting, with case management or care coordination responsibilities.
