Item 1: Events

Discussion:Provider and Health Fair May 10,2018 11:00-3:00pm

Picnic in the Park August 24, 2018 11:30am – 2:30pm

Item 2: Project Search

Discussion:Working on getting partnerships to the table. The school has shown interest, but no one has stepped up to be the instructor. Human Services Department is looking into preceding with possibly hiring a sub- contractor.

Item 3: Vote for CCM Members and Officers

Discussion: Attendees voted, and the affiliate Representative are Marilyn Kubler and Traci Burney and Consumer

representatives are Kyrin Houghton and Lorena Holguin.

Chair is Tamera Paine and Vice Chair and Secretary will be voted on next meeting.

Item 4: External Human Services provider and CCM Training site

Discussion: Goal – is to have trainings available starting January 2019. Some CCM training will be develop by CDDO staff and Provider Required trainings will be purchased. It was request that training be made available to everyone at no charge. The trainings for CCM will be free, however, the provider trainings may have a cost.

Item 5:Quality Assurance Focus

Discussion:

  • Main file Records available. (all required documentation)
  • Residential and Day records available (PCSP, BSP, Health profile, MAR, frequency and seventy tracking and goal tracking, Calendar of activities and community inclusion, proof of meal planning and budget planning. Clean and well Maintain Physical facilities.)
  • Rights –What is being reviewed?
  • ANE- What is being reviewed?
  • Human Rights Committee? Who is on the committee and how often does committee meet?
  • Specific Behavior Frequency and severity tracking
  • Goal tracking
  • Medication error tracking
  • Service being provided
  • Service agreement

Item 6: QE Focus

Discussion:

  • The Health Profiles provide a snapshot overview of health for each individual
  • Emergency preparedness (additional to fire and tornado)
  • PCSP-BSP-KAMIS –coincide
  • Frequency and severity tracking
  • Goal tracking
  • Staff files
  • Side effects of medication

Item 7: Review Policies.

Discussion: Reviewed Rights notification and Uniform Access - Recommendations are documented. The following policies will be sent to CCM and provider to read through and bring suggestions to the next meeting.

(Affiliation, Implementation, Single point of Entry, Referral for services, and Quality enhancement.)
