Wait List Policy for JACC

PURPOSE:To set forth the policy under which individuals may be classified and entered onto a Wait List for enrollment into the JACC program.

DEFINITIONS: For clarity, the following definitions are assigned as applicable to this policy:

JACC Wait List means the list of individuals who have been determined to meet both official financial and official clinical eligibility for JACC services, but who cannot be enrolled due to lack of “sufficient funding “ under the county Spending Authorization. This will be referred to as “JACC wait list” or ‘Wait List”.

Official Financial Eligibility means that the NJEASE representative has received a letter (JACC 432) from the DHSS confirming that the applicant has met resource eligibility requirements for participation in JACC.

Official Clinical Eligibility means that the NJEASE representative has received a Health Service Delivery Plan from the Long-Term Care Field Office, or possesses a completed CAT tool prescribed by the Presumptive Eligibility Process in JACC 505P, confirming that the applicant has met the level of care requirements for participation in JACC.

Date of Eligibility means the latter of the two following dates:

  • The date of Official Financial Eligibility as indicated on the JACC 432 letter.
  • The date of Official Clinical Eligibility as indicated by the Pre-Admission Screen date on page 2 of the HSDP, or the last page of the CAT in the case of Presumptive Eligibility.

Spending Authorization means that amount of money identified by the Department as available to a given county for the purchase of services to qualified individuals in the JACC program.

Sufficient Funding means that, under the Spending Authorization for a given county, program money is projected as available to purchase at least case management and one other service for the applicant for each month of the remainder of the State Fiscal Year (July 1 through June 30).

POLICY:

1) A JACC Wait List may exist for each county, subject to validation and review by OCS.

2) The JACC Care Coordinator at the Lead NJEASE Agency shall initiate and maintain the JACC Wait List.

3) A JACC Wait List may only exist pursuant to the points outlined in this policy, i.e. when there is not sufficient funding to enroll an additional applicant.

4) Alternate services shall be secured for the applicant as possible during the Wait List period.

5) Upon initiation of a Wait List in a given county, the county may elect to suspend its commitment to conduct presumptive eligibility pursuant to the process outlined in JACC 505P. Suspension and planned resumption of presumptive eligibility processing shall be confirmed by the County in prior written notification to OCS.

6) Applicants shall be added to the Wait List in chronological ascending order based on Date of Eligibility, except that enrollees transferred from another county shall precede non-enrolled individuals.

7) The JACC Wait List is officially maintained by the JACC Care Coordinator at the Lead NJEASE Agency in each county.

8) Only applicants who have met official financial and clinical eligibility may be added to the county Wait list.

9) The Wait List shall contain the following information, at a minimum:

(a) The applicant’s name, date of birth, and contact phone number

(b) The date of financial eligibility

(c) The date of clinical eligibility

(d) The date entered on the Wait List

(e) The date removed from the Wait List and reason

10) If the applicant is on the Wait List for a period of one year from the date of clinical eligibility, a reassessment of clinical eligibility must be completed prior to enrollment.

11) If the applicant is on the Waiting List for a period of one year from the date of financial eligibility, a reassessment of financial eligibility must be completed prior to enrollment.

12) Applicants shall be taken off the Wait List in the order their names appear on the list.

13) The Wait List shall be maintained and submitted with the Care Coordination data elements that are submitted to OCS monthly.

14) Documentation to support the applicant’s Wait List eligibility must be readily available at the Care Coordinator’s site.
