This is to verify that our team have already read in detail all of the related rules and regulations for the 2018 CTBC‧TAIPEI 101 Run-Up and promise that participants shall abide by and observe all of the race regulatory items. This is also to verify that all team members are in good mental and physical health, do not have any type of diseases or conditions which would not be suitable for taking part in any form of vigorous exercise. We understand and accept any risks involved in participating in the race and would like to continue the registration. In case of the occurrence of any accident or incident during the race, all team members and their family are willing to shoulder the responsibility of any unknown risks or emergency which may happen to us during the race, and hereby discharge the organizers from any responsibility in the race of our injury, death or any loss, and should not make any claim for any form of compensation; all team members grant permission to the organizers to make announcement of their result, video recordings, photo taking, the organizer can reproduce, broadcast, display, edit, distribute, exhibit, publish, present on-line or use in other ways worldwide whenever, we further agree that the organizers, organizations or companies authorized by the organizers (co-organizers or sponsors) have full authorization and permission to utilize our name, appearance or results for the website, publication, or related promotion, activity, publicity of the race. Our team guarantee that the ID numbers and personal information we submitted are true and valid for verifying their identity, and acknowledge to shoulder all of the related legal responsibilities or legal consequences. Upon submitting the registration, all team members have not the right to dispute the race execution with any reason. In case any member fails to pass the preliminary health check on the race day, the members agree to take the health check again. If the members still fail to pass the health check thrice, in consideration of the own safety, the members agree to withdraw from the race based on the medical advice without any refund. Our team hereby confirm to participate in the race after serious consideration.

□The Chinese Taipei Road Running Association (hereafter referred to as the "CTRRA") strictly adheres to the recently promulgated provisions for protecting one's personal information. Please note from the conditions of Article 8 of the Personal Information Protection Act (hereafter referred to as the "PIPA") the following: According to the related legal provisions of the PIPA, governing authorities can grant and approve an organization to collect personal information of an individual as for 001 Personal Insurance, No.142 sports activities. Categories include Personal Identification (C001), Government Information Identification (C003), Personal Description (C011), Body Description (C012) and Other Social Relations (C024). Your personal information will only be used during March 30 2018 to May 31 2018 for TAIPEI 101, CTRRA、related organizations and sponsors for the event related use. You are entitled to the following rights according to the Article 3 of the Art: make en enquiry or request reviewing the information, request a copy of the personal information，request supplementation or corrections，request stopping of data collection, handling or utilization, and request deletion. To protect your personal information, your request will be handled as soon as your identity is verified. You deserve the rights to provide the personal information, however if you cannot provide the respective personal information, you might not able to participate in this race.

Remarks：1.If any team member is over 15 years old, but under 18 years old, please download and complete theAcknowledgement and Authorization DeclarationFormwithhis/herparent or guardian’s signature. It is required to submit with the documentsmentioned in Remark2belowfor registration.
2.Please provide Corporate Team Declaration Form, Team List, Business/Unit/School Certificate, Registration Fee, as well as the“Acknowledgement and Authorization Declaration Form(For individuals aged 15-18)”in case there is anyteam memberaged between 15 and 18 years old,to the Chinese Taipei Road Running Association (CTRRA).
