Applications are invited for this consultant post, arising due to a retirement, in NHS Greater Glasgow and Clyde which will be based in Queen Elizabeth University Hospital (QUEH). The successful candidate will join a team of 8 established consultants, an Associate Specialist and a Specialty Doctor to provide care for rheumatology inpatients, day cases and outpatients. They will provide both acute and continuing care to medical in-patients at the QueenElizabethUniversityHospital.

The post offers the successful candidate the opportunity to develop a sub-speciality. The speciality is supported by Clinical Nurse Specialists and two medical day units.

Applicants must have full GMC registration and a licence to practise. Those trained in the UK should have evidence of higher specialist training leading to CCT in General (Internal) Medicine and Rheumatology or eligibility for specialist registration (CESR) or be within 6 months of confirmed entry from date of Interview. Non UK applicants must demonstrate equivalent training.

QueenElizabethUniversityHospital, NHS Greater Glasgow and Clyde

The QueenElizabethUniversityHospital is a new purpose built hospital that opened its doors in May 2015. The hospital has 1109 beds and provides provide medical care to a population of around 700,000 people in west and south Glasgow. The hospital provides the full range of medical and surgical services and many tertiary specialties including neurology, ENT and renal medicine. It will also be one of the 4 hospitals in Scotland to admit major trauma cases from next year.

Development of the new QEUH has allowed inpatient and acute medical care from the Western Infirmary, Gartnavel General, VictoriaHospital and Southern General to be consolidated to a single site.

The Work of the Rheumatology Department.

The QUEH is the base hospital for the largest department of Rheumatology in Scotland, which provides specialist in-patient and outpatient care to the population of South and Northwest Glasgow. This replacement post will join the team of 8 other consultants, Associate Specialist and Specialty Doctor to deliver inpatient, outpatient and day care service.

The successful candidate will contribute to the acute medical admissions unit as part of a receiving team with colleagues from Diabetes & Endocrinology, Infectious Diseases and Medical Academics. It is expected that initially 9 programmed activities per week will be devoted to direct clinical care, with 1 programmed activity allocated for supporting professional activities.There is potential for extra SPA activities subject to negotiation.

c)Clinical Nurse Specialists

There are five full time and two part time CNS supporting the work of the department, with two full time CNS having special responsibility for work on the QEUH site.

Details of the post:

Acute Receiving

The new appointee will have regular acute medical receiving duties on the RAID (ARU2) receiving cluster. When receiving, the consultant will be on call from Monday to Thursday daytime (08:00-18:00) or overnight on a 1 in 20 basis. Friday, Saturday & Sunday are covered by 2 Consultants sharing the 1st on/2nd on duties, with an overall frequency of 1 in 8 weekends. The 1st on receiving consultant covers the 16 bedded receiving unit, including twice daily formal ward rounds. The 2nd on receiving consultant reviews up to 14 patients that have been admitted directly to other wards during the night before and, at weekends, supports the junior/middle grade staff on upstream RAID medical wards and Medical HDU (during the week the MHDU is covered by colleagues from Critical Care).

Non-Receiving In-Patient Work:

The QEUH rheumatology team covers 42 beds in two medical wards and patients with rheumatological problems are admitted here whenever possible, alongside general medical patients. This post holder will be expected to share responsibility for covering ward 6B with 3 other rheumatology consultants and an academic physician who provides input for 6 weeks per year.

Out-Patient Workload:

The post holder will offer 4 outpatient clinics per week - a New Patient Clinic, and three return clinics. A specialist interest in Connective Tissue Disease or other sub-speciality area would be encouraged, and expertise in musculoskeletal ultrasound would be welcome. Clinics will be at the WEUH and the New Victoria and there are twice yearly satellite clinics in Campbeltown for patients with established rheumatic disease.

Day Unit Workload:

The day unit workload will be shared with colleagues from QEUH (Drs Morrison, Crosbie & Zgoda) and the New Victoria Hospital (Dr McGuchan) supporting the delivery of biologic therapies on the site.

Rheumatology referrals

The post holder and on-call rheumatology specialist trainee will review inpatients referred (during working hours, Monday – Friday) for a rheumatology opinion on one day a week.

Administration

There is 1 PA for administration duties associated with this post. SPA time will be allocated within the job plan to support the post holders personal appraisal and revalidation work

Teaching

Undergraduates attend the unit throughout their training and many opportunities exist for contributing to the medical undergraduate curriculum. There is a need for clinicians to support post-graduate training through teaching and educational supervision. Such involvement in teaching and training would be encouraged, and the department would support the development of the successful applicant’s job plan to reflect this

Continuing Professional Development

There are monthly XR meetings with a specialist musculoskeletal radiologist, weekly postgraduate teaching meetings in rheumatology (Monday 12-1pm), clinical governance (Wed 12.45 -1.30) and Hospital Grand Round (Friday 12.30-1.30). There is a weekly research meeting at Gartnavel GeneralHospital (Friday 8-9).

Research

The University of Glasgow has an international reputation for research in rheumatology. Consultants within the unit have established track records of research collaboration with the University delivering clinical trials (e.g. TICORA and ORBIT studies), cohort and biobanks (e.g. SERA and SOPHOS), and basic immunology studies. The Centre for Rheumatic Disease/University of Glasgow has received a EULAR Centre of Excellence award (one of only nine such centres in the UK), and hosts the Arthritis Research UK Rheumatoid Arthritis Centre for Excellence. Opportunities abound for collaborative research with academic colleagues (Prof Iain McInnes and Dr Stefan Siebert) who are keen to support NHS colleagues who wish to apply for NRS Career Research Fellowships. There are currently four clinical research fellows taking time out of programme with the aim of studying for Higher Degree.

SPA

The current job plan has only 1 SPA. However, the unit is keen to encourage the pursuit of excellence, and will actively support the successful applicant if he/she wishes to participate in other SPA activity including teaching, training, service development, quality improvement or research by negotiating additional time in the job plan.

Indicative Job Plan:

This job plan is negotiable and will be agreed between the successful applicant and the Clinical Director. NHS Greater Glasgow & Clyde initially allocates all full time consultants 10 PAs made up of 9 PAs in Direct Clinical Care (DCC) and one core Supporting Professional Activities (SPA) for CPD, audit, clinical governance, appraisal, revalidation, job planning, internal routine communication and management meetings. The precise allocation of SPA time and associate objectives will be agreed with the successful applicant and will be reviewed at annual job planning.

The proposed job plan is as below. It is recognised that this plan is provisional. All clinics are 3 hours of clinical activity with an hour of administrative time built in and are not shared lists with other Consultants.

The appointee will be required to register [if not already done so] with an appropriate College for CPD, undergo an annual appraisal, and subsequently revalidation as determined by the General Medical Council [GMC]. 1PA [4 hours] will be allocated to such activities.

Qualifications:

Essential: CCT in Rheumatology and General (Internal) Medicine.

Desirable: Experience in Acute Medicine

TERMS AND CONDITIONS OF SERVICE

The conditions of service are those laid down and amended from time to time by the Hospital and Medical & Dental Whitley Council.
