  1. Fully describe a crash
  2. How could this crash have been prevented? (pre-crash)
  3. How could the consequences of this crash have been mitigated? (during crash, post-crash?)

Use the Haddon* matrix

*note: It would seem you have to know the causes first, then you can propose mitigations. So in that sense, you can put either a cause or a mitigation in each box. I like to do both. However, that will not apply to all boxes, as post-crash mitigations are not responding to post-crash causes. So, in that sense, you can just use the Haddon matrix to organize your thought process and put anything you like into the boxes, as long as they pertain to the crash you are describing. There is no right or wrong specific answer to this assignment, but I will note the care you take in coming up with the material.
