GENERAL INSTRUCTIONS – UF-400, Single Student Housing 2015

A) Submit one (1) hard copy of complete typed proposal – plus one electronic copy of the same (Important: see instructions for electronic transmittal on section E of Project Fact Sheet) – by the time and date stated on the Project Fact Sheet.

B) Assemble proposals in a manner that facilitates disassembly & recycling and minimizes the use of plastic covers and dividers.

C) Include a Letter of Interest and (8) tabbed & divided sections as follows, attaching additional pages for each section as necessary:

D) Forms may be retyped or re-sized to provide additional legible information or pictures, provided that the requested information is provided, and in the order requested. These Instructions need not be submitted.

E) Number each page consecutively, including the letter of interest, CMQS forms, questions & answers, and all attachments, licenses, résumés, supplemental information, etc. The entire proposal shall be limited to twenty (20) double-sided 8½” X 11” pages. Covers, table of contents, and divider tabs will not count as pages, provided no additional information is included on those pages. Any pages over the stated limit will not be considered as part of the application.

F) The Letter of Interest should concisely outline both your understanding of the UF-400 project and the characteristics of your firm(s) and proposed team that make them uniquely qualified for it. Address the letter to the attention of the UF Project Manager.

G) Use the project-specific CMQS 0 form to provide general company information and to formally certify the proposal.

H) For CMQS 2, use the enclosed form and follow the Submittal Instructions. For the questions in CMQS 3-5 and CMQS 6-8, precede each answer with a re-statement of the given question. If the question on joint ventures (CMQS 12) does not apply, simply state “NOT APPLICABLE.”

I) Enclose resumes, LEED accreditation, and other pertinent credentials for all proposed staff.

J) Applicants operating as a corporation, limited liability company, or partnership must be registered to operate in the State of Florida by the Department of State (Division of Corporations) at the time of application. Provide proof of such status (if applicable) for the applicant. Such proof shall take the form of a Certificate of Corporate Status from the Florida Department of State and the Certificate must be issued by the Department of State within ten (10) calendar days of the application submission date.


K) Enclose a copy of the applicant’s current general contracting license from the Department of Business and Professional Regulation.

L) Provide proof of the builder’s Experience Modification Rating (EMR) for the past three years as determined by the National Council on Compensation Insurance (NCCI). Such proof shall consist of copies of the firm’s NCCI EMR forms.

M) Provide a letter of intent from a surety company indicating the applicant's bonding capacity for this project. The surety shall acknowledge that the applicant may be bonded for each phase of the project, with a potential maximum construction cost of $20,000,000. The letter of intent shall confirm that the Surety Company is licensed to do business in the State of Florida, has a Best Rating of at least "A,” and has a financial size of "Class XV."

N) Provide proof of the applicant’s ability to provide liability insurance coverage in the amounts of $1 million General Liability and $5 million Umbrella.

O) Only individual firms or formal joint ventures may apply. Two firms may not apply jointly unless they have formed a joint venture, and a copy of that formal agreement shall be provided with this proposal. See CMQS 10.

P) Applicants should take care in assembling the proposals, as professionalism and attention to detail are among the intangible qualities the Selection Committee will consider.

Q) Incomplete proposals will be disqualified. This includes, but is not limited to:

§  Failure to submit proposal by the stated time and date

§  Failure to sign the CMQS 0 certification form

§  Missing, invalid, or expired license

§  Missing, invalid, or expired proof of corporate status in the State of Florida

§  Insufficient proof of bonding capacity or insurance coverage

§  Failure to include safety (EMR) data

SUBMITTAL INSTRUCTIONS – UF-400, Single Student Housing 2015

NOTE: The numbers on these instructions correspond to the numbered items on the University of Florida Construction Manager Qualifications Supplement (CMQS) forms.

1. APPLICANT'S PERSONNEL

Name all personnel who will be part of the pre-construction and construction management team for this project. Identify each person’s title, project-specific responsibilities, and city of residence. Also identify other non-essential positions planned for this project that are not, as yet, filled or named. Enclose résumés for all key staff and discipline heads tailored to demonstrate their experience as it relates to this project. If any consultants are included as part of the proposed team, provide resumes for the specific personnel proposed for this project and describe each consultant's proposed role in the project and its related experience. Provide an organization chart or diagram if necessary to clearly explain lines of authority, duties & responsibilities, continuity through design, construction, and post-occupancy, etc.

The team proposed in the CMQS must be available to provide the services for the project, throughout the project. If the Applicant discovers prior to the interview that any part of the listed team (either individual key staff or consultants) will not be available, it shall notify the selection committee immediately. The selection committee will determine whether the change in the team would have affected the Applicant's shortlist score. If the change would lower the score, the Applicant may be removed from the shortlist. Once awarded the contract, the Applicant will not be permitted to alter its team without the Owner's approval.

2. EXPERIENCE / REFERENCES (use attached form)

Submit up to ten most related projects for which the Applicant provided (or is providing) CM, GC, D/B, or other construction management services. In determining which projects are “most relevant” to the UF-400, Single Student Hosusing 2015 project, consider the unique aspects of this project, including architectural feature requirements and historic characteristics, location and sensitivity to the adjacent buildings and visibility from the main roads, phases of the project, the delivery method, the demands of constructing on a bustling campus, and other project-specific issues as conveyed in the selection criteria outlined in the Project Fact Sheet. Also consider the Owner and location, relative size and cost, building/space types, complexity, staffing (how many members of the proposed team worked on the listed project?), currency (how recently was the listed project completed?), and performance metrics (schedule, budget, quality).

Use the attached form (one two-page form for each of the ten projects) and provide all requested information as follows:

§  Note the title and location of the project; indicate the services provided; and complete the other general project information (status, size, value, delivery method, etc.).

§  Provide the names of all key staff – whether construction, design, or both – and check YES or NO to indicate whether each person is also on the proposed team for the UF-400, Single Student Housing 2015 project.

§  Provide brief narratives on the facility and its relevance to the UF-400, Single Student Housing 2015 project.

§  Provide contact information for the Owner, designer or builder (whichever applies), and the (3) largest subcontractors by value of trade package.

§  Verify that the contact information is still valid (names and numbers). Add, where applicable, contact information for the current Owner’s representative, if different from the Owner's representative at the time the applicant’s services were provided.

List the projects in priority order, with the most relevant project listed first and the least relevant project listed last. INCLUDE NO MORE THAN 10 TOTAL PROJECTS.


CONSTRUCTION and PROJECT MANAGEMENT QUESTIONS

3. QUALITY CONTROL and CONSTRUCTABILITY

·  Explain how constructability, fit & finish, means & methods, and other construction issues are considered during the design phase. For example, are the personnel responsible for designing and estimating the project design also responsible for providing constructability, coordination, and true “Value Engineering” input to the Owner and design team?

·  What means are used to cross-check design submittals against the UF & DHRE Design & Construction Standards, previous review comments, Value Engineering input, and other direction or feedback?

·  Will the builder review design submittals with an eye for errors, discrepancies, conflicts, and other potential change order issues? If so, who specifically does that?

·  Describe the methods used by the team to maintain quality control during the construction phase. , particularly if the applicant is a contractor-led teaming of design and construction firms.

4. COST ESTIMATING and CONTROL

·  Describe the team’s approach to, and methods for, cost estimating during the pre-construction phase and cost control during the construction phase.

·  Elaborate specifically on cost estimating methods for CSI Divisions 15 (mechanical, plumbing, fire protection) and 16/17 (electrical, telecommunications).

·  What means are used to accurately estimate costs during early design phases when material specifications, systems, and fabrication details are not yet developed?

·  Discuss how pre-construction estimates are carried over from one design phase to the next (i.e., is the project re-estimated with each design submittal, or are baseline numbers increased or decreased as needed?) and at the same time from Phase I construction to phase II construction.

5. SCHEDULING

·  Describe how the team will develop and maintain the construction schedule throughout design and construction, specifically, phase I & Phase II. Who develops the schedule during pre-construction? Who develops the schedule that accompanies the GMP proposals? Who is responsible for updating and adhering to the schedule during construction phases?

·  Given your understanding of the Owner’s goals, requirements, and constraints, elaborate on ideas you have for “fast-tracking” the project without sacrificing quality or introducing unreasonable risk.

·  How exactly will typical North Central Florida weather be accounted for in the schedule?

·  Given your knowledge of how UF strives to handle the construction completion process – including Commissioning, training, record documents, O&M documents and other closeout deliverables – describe how this is properly accounted for in the schedule.

6. BIM

Building Information Modeling (BIM) is now the standard for work at UF, but the University is interested in the power of this technology beyond assisting with design phase visualizations and conflict resolution.The Project will utilize Building Information Modeling (BIM) as a technology for improving quality, cost, reducing field conflicts, and many others. Outline the team’s experience with Building Information Modeling. In doing so, specifically identify the experience of each design discipline’s staff; describe the extent to which BIM will be used during the design and construction of this project; and Explain the team's approach to maintaining and updating all models through and following construction to ensure that the record ("as-built") models are accurate, complete, and inclusive of data/information for the actual products, equipment, and systems installed.

7. Communication, record-keeping, and document control are critical aspects of a well-managed project. Identify the specific person or persons who will be responsible for the following tasks or actions and briefly outline their experience with same:

§  Meeting minutes

§  Cost Estimates (pre-construction)

§  Invoices (pre-con) and applications for payment (construction)

§  Schedule development (pre-con) and schedule management (construction)

§  Procurement of subcontractors (pre-qualification, advertisement, bid packages, scoping, etc.)

§  Change Order Proposals

§  Owner Direct Purchase

§  Requests For Information (pre-con) and Requests For Information (construction)

§  Submittals and shop drawings

§  Testing and inspection reports, plus action items stemming from same

§  Quality control/assurance

§  Construction phase photography

§  LEED-related documentation

§  Rebates information

§  Safety

§  Commissioning meetings, correspondence, actions, etc.

§  Substantial Completion documents, including punchlist(s)

§  Closeout documents, including model and red lined as-builts, O&M manuals, and Owner training documents

§  Warranty-related action items (post-occupancy)

8. COMMISSIONING and LEED

a.  Describe the experience of the proposed field staff with building commissioning.

b.  Describe the experience of the proposed project management staff with sustainable construction and LEED certification efforts. Enclose copies of proposed staff’s LEED accreditation(s).

9. What specific issues or challenges does the CM team see as most important to resolve?

10. Outline your ideas for the specification, detailing, and procurement of a Variable Refrigerant Flow (VRF) mechanical system in a way that maximizes opportunities for open, competitive pricing when bid.

11. Given the scope and goals outlined in the Facilities Program and Owner’s Project Requirements, the site itself, the current North Central Florida construction market, the anticipated timing for procurement and construction, and other project-specific factors … discuss the construction budget for this project and identify potential challenges and opportunities for maximizing the value & performance of the facility.
