APPLICATION FOR CONSENT TO IMPORT UNAPPROVED ACTIVE CONSTITUENTS OR UNREGISTERED AGRICULTURAL OR VETERINARY CHEMICAL PRODUCTS UNDER MISCELLANEOUS CIRCUMSTANCES

Prior to submission of this application, applicants must be

familiar with the information below.

Applications for Consent to Import must be lodged by a person who is either a resident of or is carrying on a business in Australia.

The Consent to Import issued by the APVMA, is generally only valid for importations during the two-week period either side of the estimated date of importation and only covers the importation specifically applied for. Should there be any changes to the information provided, a new application is required. If unable to estimate an exact date of importation, the month of the expected importation is to be nominated.

Circumstances under which this application form may be used include:

·  Importation of a product to be market ready when registration is granted. All registration fees must have been paid and there must be no outstanding technical obstacles to registration. Applicants should be aware that the APVMA would issue show a cause letter advising the intention to issue a compulsory ‘not supply’ notice under s101 of the Agvet Code. Such notices are required to be published in the APVMA Gazette. Importers would have on-going reporting responsibilities and the product would have to be securely quarantined until registration is granted.

·  Limited promotions or events, where the product will not be supplied in Australia and after which the product will be either re-exported or destroyed. Evidence of either outcome will need to be supplied.

Applicants should be aware that some commodities require additional approvals from agencies other than the APVMA. It is the applicant’s responsibility to check with these agencies. Agencies include:

·  Therapeutic Goods Administration

·  Office of Chemical Safety and Environmental Health

·  Office of the Gene Technology Regulator

·  Australian Quarantine and Inspection Service

·  Department of Agriculture, Fisheries and Forestry

·  Department of Foreign Affairs and Trade

·  Department of Sustainability, Environment, Water, Population and Communities

I declare that:

·  I have read and understand the information on this page

·  the information provided in this form is complete and correct.

I understand that:

·  providing false or misleading information is a serious offence.
