I understand that although all reasonable precautions and due care will be taken; there is always a potential risk with anesthesia and surgery. I accept these risks and authorize White House Animal Hospital to perform such treatment as deemed necessary. What you should know about anesthesia and your pet.

Because there is always the possibility that a physical exam alone will not identify all of your pet’s health problems, we strongly recommend that a pre-anesthetic profile (a combination of tests) be performed prior to anesthesia. To ensure your pet can properly process and eliminate an anesthetic, we run these tests to confirm that your pet’s organs are functioning properly, and to find hidden health conditions that could put your pet at risk. The cost for this is $46.25for blood chemistry or $73.02 for blood chemistry and a complete blood count.

Both hospital policy and state law require that pets be current on Rabies vaccinations. If you are unable to provide us with written proof, one will be administered to your pet. The cost for this with aWellness examination is $59.82______.
