Thisformmustbesignedbyboththestudentandaparentorguardianbeforethestudentcanuseacomputer.Theformwillbekeptonfileforthecurrentsemesterandisintendedtobeusedforeachcourserequiringcomputeraccesseventhoughnotalltheitemsbelowarerelevantforeverycourse.
IssuesResponsibilities:TheInternetcontainsawealthofinformationaswellasmaterialthatisobjectionablefrommanypointsofview.PleasenotethattheUniversity’scomputerlabs areunfiltered.Instructorswillcloselymonitorstudentuse.EachstudenthavingInternet access willparticipateinadiscussionwithhisorherinstructorregardingacceptablebehavioranduseofthenetwork.Whererelevanttothecourse,studentswilldiscusswiththeirinstructorpotentialdangers,threats,andconsequencesthatcanarisewiththeuseofsocialmediasites.Internetaccessistobeusedonlyforteacherdirectedactivities.Ifastudentisinviolation,hisorherInternetaccesswillbeterminatedandfutureaccesscouldbedenied.
