COMPLETING THE RISK ASSESSMENT, FORM PPS 2030D

The Risk Assessment should be completed on any child in the home. The Risk Assessment is to assess the risk of future maltreatment. Complete this form only for child abuse/neglect cases.

CASE NAME: Use the same case name as on page 1, Section II, #1 of form PPS 1000.

CASE NUMBER: Use the same case number as on page 1 of form PPS 1001.

EVENT NUMBER: Use the same event number as on page 1 of form PPS 1001.

SOCIAL WORKER: Enter the name of the assigned worker from form PPS 1002, page 2, Section III.

DATE OF ASSIGNMENT: Enter the date of case acceptance from form PPS 1002, FC Educ - tracking recordspage 1.

Section I: PAST ABUSE, NEGLECT, AND OTHER GENERAL RISK FACTORS

Indicate the type of abuse or neglect in the current report. Check any statements that are true.

Abuse includes: physical abuse and emotional/mental abuse

Sexual abuse includes: sexual abuse only

Neglect includes: physical neglect, lack of supervision, and abandonment

Service History means case was previously opened for services.

Describe any past Abuse/Neglect or service history from Section I in narrative. If there has been in the past, or is currently an assessment involving a child death as a result of abuse or neglect by a current care giver, check yes.

Section II: CHILD RISK FACTORS

Complete for all children identified in the family. Identify child by name and age. Check any factors identified based on the assessment of the family.

Section III: PARENT/CAREGIVER RISK FACTORS

Complete on all caregivers in the home. Identify caregiver by name and age. Primary caregiver provides primary care for the children. Secondary caregiver provides secondary care for the children.

Other risk factors: Check any that are identified within the assessment of the family. Complete on all care givers in the home.

Section IV: NARRATIVE AND ANALYSIS

For each risk factor identified in Section III, or other risk factors not noted above; describe each section of risk factors and how it affects the child's safety. Also, describe the family's ability to use strengths and resources to manage the risk factors.

Describe any other significant family or child strengths or resources not noted above.

Section V: RISK ASSESSMENT CONCLUSIONS

Risk level is computer generated for abuse and neglect. The risk level is computed at low, moderate, high or intense. The highest risk level is the overall risk level.

If an override is identified the overall risk level is automatically intense.

Overrides include:

non-accidental physical injury to an infant

non-accidental physical injury requiring hospital or medical treatment

if sexual abuse, perpetrator is likely to have access to the child

 there was child death as a result of past abuse or neglect

Risk Conclusion: Risk conclusion is determined through the assessment of risk factors identified for children and caregivers in the home, family strengths and resources to control the risk, and the overall risk level.

Conclusion choices include:

No significant risk: There were no significant risk factors identified.

Risk controlled: Significant risk factors were identified; but family strengths or available resources are sufficient to provide for the child(ren)'s safety.

Risk present: Risk factors were identified and there are NOT sufficient family strengths and available resources to provide for the child(ren)'s safety without agency intervention.
