COMMUNIVERSITY PARTICIPATION AGREEMENT

Douglas County Day Center is here for YOU!! We want to provide classes, activities and self-directed engagement materials that are interesting and fun, as well as opportunities for learning, employment, volunteering and community involvement. Please read the guidelines for participation in classes & activities and initial on the sign-in sheet to indicate that you’ve read and agree to these guidelines listed below:

  1. Document each visit on the sign-in sheet. We want to know you were here!
  1. Please RSVP in advance for classes and special activities (e.g., parties) prior to participation so that we are prepared with adequate supplies and space. If you RSVP for a class/special activity and then cannot attend, please email as soon as possible to let us know that you won’t be coming after all.
  1. Always stay with your caseload of individuals and provide them with the supervision and support that they need. IF you need to go to the restroom or do something important briefly that will require you to be away from your caseload, please do a formal pass-off to (a) one of your co-workers, (b) one of the day teachers, or (c) with a Day Services Supervisor.
  1. Please assist your individuals to participate in all classes/activities. There likely will be a class/activity leader BUT we need YOU to help your individuals participate and enjoy the class/activity as much as possible. Keep your focus on teaching & interacting with your caseload. Stay off cell phones.
  1. Come dressed as appropriate for participation – weather and types of activities can dictate type of dress needed. ALWAYS where close-toed shoes.
  1. IF you are unsure where self-directed engagement supplies can be found, just ask!
  1. Always help clean up after classes/activities/lunch.
  1. If anyone in your group uses a walker, please make sure it has proper “footies” to ensure it doesn’t scratch the floor. The Day Supervisor plans to have extra in her office; can also ask Karen Fenza.
  1. When at the Center, remember that the Day Services Director & Assistant have functional authority.
  1. DC Day Center hours are primarily from 9 to 3. IF dropping off before then, you need to stay with them unless there’s an available day service staff to whom you can pass off.
