Community Resource Survey

Dear Community,
The Western Mass Recovery Learning Community (RLC) is working to create a resource book and database. It will cover all of Western Mass (and relevant statewide and national resources) pertaining to wellness and recovery. Whether you are a person who has lived experience with psychiatric diagnoses, extreme states or trauma or you are working in a provider role, we need you to help us find out about the resources that should be included!
Please take a moment to complete the survey below and share your thoughts and the wisdom of your experience to help us to build this important community offering. Please note that, for the purpose of this survey, a ‘resource’ could be anything that helps people in our area create and live healthy, secure and balanced lives including mental health agencies, support groups, housing, meditation groups, free or low-cost yoga classes, advocacy supports and beyond.
We request that you return this survey to us by Monday, May 23rd. You may return the survey by e-mail ( – and electronic copy is available on our website at mail (187 High Street, Suite 303, Holyoke, MA01040) or by phone (413 539.5941 ext. 3xx). Please note: Although we are requesting this information by May 23rd for inclusion in the first draft of our guide, please also feel free to submit additional resources on an ongoing basis for future revisions. Please be sure to include as much detailed information ( name, number, address, e-mail, website, etc.) as you are able to when submitting any resources! Use extra paper as needed!

essential to your wellness and recovery?
B. If you are a person who provides services, about which resources have you
received the most positive feedback?
- What types or resources have you had the most trouble finding when you’ve looked for them? What resources do you wish were more available or that you knew more about?
- What resources have you learned about that you don’t often seen listed in resources guides? (Can include anything from a local support group to a place to find rental or housing support, etc.)
- Are there any on-line resources that have been really helpful to you? (websites, newsgroups, discussion groups, etc.)
- A. If you are someone with lived experience, are there any books or films that
have really supported or helped you in your own path to recovery and wellness?
B. If you are in a provider role, are there any books that have really helped you
learn about other perspectives and/or understand how to be a better support?
