PERFORMANCE EXPECTATIONS:

Related to Home Health and Hospice Programs Operations

1. May accept requests for services and reviews the service need with the scheduling person.

2.  Oversees scheduling and/or participates in scheduling to ensure delivery of high quality services in the most efficient, effective and economical manner.

3. Serves as a resource to staff regarding the utilization of community resources as they relate to the planning of home health or hospice client care. Assists with coordination of these resources/models. Resolves or approaches identified issues/problems using critical thinking behaviors or effective problem solving/decision making.

4. Assesses and evaluates potential client/families for appropriateness, or hospice or home health services, and/or telehealth services.

5. Conducts initial informative visit with client/family to explain Medicare Hospice Benefit, or delegates the responsibility to appropriate staff.

6. Acts as core member of Hospice IDT. Confers with another member of IDT before providing hospice services. Provides clinical update to IDT when primary nurse is unable to attend.

7. Plans, coordinates, and facilitates Home Health and Hospice interdisciplinary meetings on regularly scheduled basis. Authorizes in conjunction with Area Manager and IDT members, services and supplies in the delivery of hospice and home health care.

8. Assists in evaluation of home health and hospice services presently being provided in the service area and makes staffing recommendations to the Area Manager.

9. Conducts a review of clinical record documentation at or above minimum standards to assess and ensure case management and documentation competency according to Agency and regulatory requirements.

10. Participates in audits and audit reports as requested by the Area Manager.

11. Assesses and evaluates service delivery and staff needs in the area and makes recommendations to the Area Manager re: staffing needs, opportunities for improvement and trends in direct service staff requirements.

12. As delegated by and in consultation with the Area Manager, interviews, administers competency evaluations, and selects candidates for direct service positions.

13. Reviews and signs weekly time reports.

14. Approves and coordinates vacation time requests.

15. Monitors staff compliance with the record-keeping system and is responsible for 60-day continuous review.

16. Coordinates discharge planning with local health care facilities and pre-discharge hospital visits when needed. Coordinates hospice inpatient services for clients on the Medicare Hospice Benefit.

17. Coordinates and supervises IV and other high tech referrals.

18. Coordinates direct service staff caseloads to facilitate quality, efficient service delivery to client population.

19. Assists the Area Manager in monitoring services delivery statistics in both the home health and hospice programs, as appropriate.

20. Coordinates and conducts utilization review/team conference meetings when appropriate.

21. Provides direct service according to program need and availability of time.

22. Attends selected supervisory meetings.

23. Helps to ensure delivery of high quality home health and hospice services in the most efficient, effective, and economical manner.

24. Facilitates an interdisciplinary approach to client care in the Home Health Program.

25.  Assures that Agency, Home Health and Hospice policies and procedures are followed.

26.  Collaborates with telemedicine nurse to support and ensure quality telehealth services to eligible area clients.

27.  Understands telehealth program objectives, client inclusion/exclusion criteria.

28.  Promotes telehealth to internal staff, physicians, and communities.

Related to Staff

1. Supervises the Area Office orientation of new staff utilizing the Nursing Competency CheckList and the HHA Skills CheckList as appropriate.

2. Provides regularly scheduled supervision of direct service staff members to assist and guide them in the development of client care plans and the performance of their duties.

3. Regularly reviews all care plans and makes recommendations, as appropriate, to the primary discipline.

4. Educates and instructs in preventative, rehabilitative or palliative measures appropriate for the client’s care and safety.

5. Assists direct service staff in evaluating care of clients and families.

6. Collaborates with the Area Manager and the Intake Department in referral coordination and case assignment to direct service staff, and coordinates interdisciplinary case conferences.

7. As delegated by the Area Manager, and with input from the Clinical Coordinators, completes probationary and annual performance evaluations and staff development plans for direct service staff, and identifies areas in need of growth.

8. Conducts regularly scheduled home visits to supervise direct service staff performance at or above minimum standards, and to assess 100% of key clinical competency areas annually, including compliance with Agency policies.

9. Is responsible for coordinating distribution of bag supplies as well as personal protective equipment to all direct service staff (including homemakers in some offices).

10. Facilitates communication between staff and administration; monitors outcome of communication; and effects conflict resolution, group facilitation, and team building.

11. Seeks and stimulates others to seek new knowledge for the benefit of clients, families, team members, and own professional growth by making continuing education/staff development needs of staff and self known to the Area Manager, and by coordinating CPR classes, updates, and the development of the office educational resource center.

12. Provides leadership and promotes a positive Agency image to staff.

13.  Directs direct service staff toward appropriate use of community and Agency resources.

Related to Program Management

1. Assumes administrative responsibility for Health Services Area Office in the absence of the Area Manager.

2. Participates in the development and revision of department clinical and administrative policies and procedures as relates to professional and paraprofessional staff.

3. Make recommendations on budgetary matters related to staff and program needs to Area Manager.

4. Assists in planning for professional students coming to the Agency.

5. Demonstrates working knowledge of the requirements of the regulatory and accrediting organizations.

6.  Assists in IOP activities for the Area Office, Department, and/or Agency.

Related to Community Relations

1. Represents the Department and Agency at community meetings as delegated by the Area Manager.

2. Assists with community public relations and publications.

QUALIFICATIONS:

To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. The requirements listed below are representative of the knowledge, skill, and/or ability required.

1. RN licensed to practice in the State of Maine, BSN preferred.

2. Demonstrated excellence in clinical practice.

3. At least five years of recent clinical experience in nursing, one year of which must have been with a licensed home health agency, and demonstrated leadership qualities.

4. Home Health supervisory experience preferred.

5. Successful passage of CHCS physical examination.

6. A current State of Maine drivers license and a reliable means of transportation with a minimum of $100,000/$300,000 auto liability insurance.

JOB PERFORMANCE EVALUATION:

Employee must satisfactorily complete a 6-month probationary period. At that time, the employee may be granted regular status or may be continued on a probationary basis for an additional 6 months. Subsequent job performance evaluations will occur on an annual basis.
