I.  BACKGROUND INFORMATION

Jenna is an eleven year old female in the sixth grade. The Gray Oral Reading test – 5th Edition was administered on January 17, 2013 in order to assess her oral reading rate and accuracy, oral reading comprehension and total oral reading ability.

II.  OBSERVATIONS

Jenna exhibited poor posture and low tone throughout the evaluation. She displayed poor enunciation, excessive head movement and finger pointing to each word as she read. She also displayed a low volume of speech and needed prompting to speak louder.

III.  SUMMARY OF SPEECH/LANGUAGE EVALUATION

The Gray Oral Reading Test – Fifth Edition (GORT-5) was administered to assess Jenna’s oral reading rate and accuracy, oral reading comprehension and total oral reading ability. The GORT-5 also provides an analysis of oral reading miscues. Five types of miscues are analyzed: meaning similarity, function similarity, graphic / phonemic similarity, multiple sources, and self-correction. Jenna had difficulty with all areas. The scores for rate, accuracy, and comprehension are combined to form an overall reading quotient. A summary of Jenna’s performance on the GORT-5 is presented below.

III IMPRESSIONS AND RECOMMENDATIONS

The results of the GORT-5 indicate that Jenna’s scores were below the level expected for a child her age. All scores (accuracy, rate, fluency and comprehension) were all described as below average. Jenna’s overall reading quotient indicated poor performance. Her percentile rank of 7 indicated that 93% of the individuals her age performed at a higher level on this measure. These results, combined with information observations and deviations from print, indicate a need for enrollment in therapy. Therapy should focus on literacy, emphasizing comprehension, rate/ fluency and accuracy.
