6. Describe the work assigned to this position, listing the critical duties and responsibilities first. Explain work in familiar terms and include machines or equipment used. Use additional paper if needed.
Responsible to work with chemical abusers in a residential setting developing new rehabilitative information services.
Documents and maintains in files the progress clients have made towards accomplishing the goals and objectives delineated in the treatment plan in a legible manner (readable and understandable to supervisor, Agency Director, licensing and certification officers). All documentation to be done daily (exception requires written approval of department director).
Completes and maintains the following forms:
-progress notes
-treatment contracts
-psycho-social history
-medical history
-staffing admission, update and discharge summaries
-PCPC forms
-appropriate letters
-all other forms as necessary to maintain client files to maintain agency, licensing and accreditation regulations
Listens to clients’ problems in order to advise, support, and guide them in solving problems and acquiring and testing new behavior patterns and skills necessary for independent living. Schedules appointments on a regular and timely basis. Required to operate audio-visual equipment. Counseling may be done with individuals, families or groups.
Provides individual counseling a minimum of two times per week for a total of at least two hours. These sessions shall be documented. This shall be done at regular scheduled intervals.
Confers on service and treatment goals in weekly meetings with treatment staff in order for coordination and implementation of individual treatment plans and approval of documents.
Assists and arranges for any ancillary services needed by client, by contacting the Case Manager for the Agency.
Transports clients using the agency van to other professional services appointments which take place away from the residential living facility. This includes transportation to AA/NA meetings.
Monitors and documents the activities of the clients in maintaining a clean and orderly residence. (i.e. cleaning rooms, chores, etc.)
Attends educational conferences in order to enhance knowledge, skills, abilities and attitudes necessary for professional advancement and will document the same.
Responds, as necessary, in emergency situations (i.e. call police, fire department, initiates fire drills, calls ambulance, etc.) in order to insure the safety and welfare of the clients.
Informs clients of inpatient rules, regulations, policy and procedures.
Reports daily at shift change any situations that will assist the incoming counselor in working with the clients.
Required to work assigned shifts in order to provide coverage for the 24-hour inpatient facility.
Required to do cooking and light housekeeping duties on some shifts, as needed.
Provides whatever services to clients are deemed necessary for proper function of ADAS services.
Provides whatever services are needed for the well being of the clients.
Performs other related duties as assigned.
