STATEMENT OF PURPOSE

To consider questions and provide input on issues that are critical to the design of a respite and carer support system that is effectively integrated within the aged care system and which is flexible and responsive to the needs of carers. This will primarily involve consideration of:

·  Respite services delivered under the NRCP;

·  Respite (under HACC service group one) and Centre-based day care (under HACC service group four), and carer counselling, information and advocacy (under HACC service group two) currently delivered under the Commonwealth HACC program; and

·  The role and structure of residential respite care, as well as its interface with home and community cased respite services and programs.

STATEMENT OF PURPOSE

The Sub-Group will examine the nature of the service types under service group 2 of the commonwealth HACC Program and other Government funded aged care programs and their role in the future CHSP program. This will primarily include consideration of:

·  The definition and interpretation of client care coordination and case management;

·  The role of assessment in CHSP and the interface/linkages to the Gateway; and

·  The role of information, advocacy, counselling and support.

The work of the Sub-Group will help inform the funding arrangements for HACC service group 2 from July 1 2015 and will be considered in the Government's consideration of transitional arrangements for 2014-2015.

STATEMENT OF PURPOSE

To provide expert advice and input to the review of the role of community transport as part of an aged care system that is responsive to consumer need. The Sub-Group will consider the issues generally relating to the role of transport services under the new CHSP, including:

·  The role and relationship of HACC funded transport with other forms of transport including public transport and other community transport; and

·  How the CHSP links to residential aged care transport services.

ASPIRATIONS

The following aspirations are in their developmental stage by the transport sub group and will be subject to further review at their next meeting in June 2013. The aspirations envisage a Community Transport system which:

·  Recognises that Community Transport provides a vital service to older people who can no longer drive or use public transport alone, or live in areas of public transport disadvantage.

·  Recognises that transport is essential to accessing the services necessary in supporting community living, such as those that address ‘clinical/medical’, ‘daily activities’, and ‘social connection’ needs.

·  Provides easily accessible and clear information on transport options in local areas to enable consumer choice (where available), and in which CDC principles are adopted by community transport providers.

·  Recognises that choice may be limited in many rural and regional areas.

·  Acknowledges values and supports the crucial contribution made by volunteers in the delivery of Community Transport services.

·  Requires funding models that to support a financially sustainable sector in both metropolitan and regional settings, and support diversity in service models to best suit local conditions and adequately met needs.

·  Embraces and supports diversity and collaboration between providers; and innovation that uses best practice models and latest technology, subject to appropriate funding. .

·  Reports all transport services, including consequential transport services (i.e. transport that occurs as part of services from non-transport specific funding) in minimum data and other data reporting practices.

STATEMENT OF PURPOSE

To provide expert advice and input on the review meal and other food services as part of a comprehensive aged system. In addition the group will also provide input into the development of future models of meal and other food services in the new CHSP.

ASPIRATIONS

The discussions from the first meeting identified some issues and aspirations to help shape the work of the sub-group. These aspirations envisage a meals and other food services system which:

·  Has a nationally clear and consistent purpose and program structure, but which is still locally responsive as well as flexible to meet individual needs, preferences and goals;

·  Provides consumers with information and empowers them to make decisions that are respected – both in regards to nutritional choices and what services are right for them;

·  Recognises the significant contribution of volunteers to the meals services and develops appropriate service models that are adaptable to changes in future volunteering patterns in the future;

·  Retains its primary focus of providing meals to those who are nutritionally-at-risk, while respecting and enabling the wide range of associated benefits around meals provided to consumers;

·  Is inclusive of the full range of meal and other food services (i.e. centre meals, group meals, food preparation, shopping, café meals, innovative programs, cooking classes etc.) not just home-based and centre-based meal provision;

·  Responds to the needs of vulnerable people, people with specific and cultural needs, and low socio-economic groups;

·  Considers the most appropriate funding model/s to ensure a sustainable meals sub-sector, including consideration of supplements, block funding, client co-contributions, non-commonwealth government contributions and true unit costs;

·  Acknowledges the benefits and importance of regulation/OHS/food safety in providing a high quality service but that recognises the impact to the sector in meeting compliance costs;

·  Acknowledges the role of dieticians and nutritionists (currently funded under SG3 – Allied Health) and tries to consistently utilise these specialists where possible/desirable; and

·  Continues to encourage localised responses, with communities of knowledge sharing and the uptake of best practice examples.

STATEMENT OF PURPOSE

To consider questions and provide input on issues regarding home maintenance and home modifications as part of a comprehensive aged care system. In addition to conducting a review of home maintenance and modifications and their role in the new CHSP, the Sub-Group will also examine:

·  The provision of goods and equipment and linen services; and

·  The role of assistive technologies and their role in the new CHSP.

ASPIRATIONS

The following aspirations are in their developmental stage by the HACC Service Group 5 Review sub group and will be subject to further review at their next meeting in June 2013. The subgroup identified aspirations for the services types under this review (this includes Home Modification, Maintenance, Equipment and Linen) which envisage a service system which:

·  Delivers Home Modifications, Maintenance Services (HMMS) and equipment services within a wellness and re-ablement approach to lowering an individual’s care needs, using a consumer directed care approach.

·  Ensures appropriate aids and equipment are available to improve quality of life

·  Allocates HMMS services in accordance with nationally consistent assessment tools;

·  Matches HMMS services to individual care plans.

·  Respects consumer choices.

·  Delivers HMMS services to a high quality standard in accordance with relevant regulations and legislation.

·  Develops care plansthat include holistic assessment via the Gateway to avoid service silos, including identification of needs and solutions and ensures holistic assessment to determine if mondification is the most appropriate service required. .

·  Has a clear, consistent national guideline that identify how and when HMMS’s are to be provided and the relationship of the CHSP with other programs.

·  Evaluates all services, and engages rigorous data collection to ensure national consistency.
