Coaches, Managers and Players guide to Academy Forms
Players
- It is the parent’s responsibility to register the player each season (Fall and Spring) and to request the Academy Player Form from their Home Soccer Association’s registrar.
- Your Home Soccer Association is determined by the bylaws of that association, for some it is school district for others it is based upon where you live (Arlington Soccer Association is based upon address).
- Parents will be required to provide a copy of the player’s birth certificate. It may be scanned and email to the home association registrar or faxed. The birth date will be confirmed, a note made on the players profile, and then birth certificate will be deleted or destroyed.
- Once a parent registers a child with their child’s home soccer association and provides a copy of the birth certificate they can call or email the registrar and request that the Academy Player Form (PAL Form) be completed and then either faxed or emailed back to the parent and/or coach/team manager.
- When registering your child with MSA, if they are a returning player, please be sure to use their Sports Pilot username and password. If not a new profile will be created and there will be no record of what recreational team they were on (if trying to return to team) nor will there be any record of a birth certificate being presented. This will slow down the process of getting an academy form, as the two accounts must be identified and merged.
- If you do not know your players, Sports Pilot username and password, please contact the MSA registrar and she will be able to email you a reset.
Coaches
- A coach may send a list of players they expect to receive Academy Player Forms from to the home soccer association registrar(s). Once those players register and the permission has been received from the parent, those forms can be emailed to the coach/team manager. However, please be aware of the time it takes to process an entire team – please allow at least 2 weeks before your event if you wish for the registrar to help gather and send Academy Player Forms (PAL Forma) and paperwork.
- Please always remember coaches that the information you receive from players is confidential. Always treat their documents (especially copies of birth certificates) with the care you would want someone to treat your own personal information.
- Coaches are ultimately responsible for insuring that all of the players that he/she rosters are properly registered.
Managers
- Managers usually bear the burden of collecting, organizing and providing Academy Player Forms to the officials who request them.
- Managers are also usually the ones that prepare the rosters for both league play and for tournaments. Please help the coach insure that all players have the proper paperwork at the beginning of each season.
- Please always remember, team managers, that the information you receive from players is confidential. Always treat their documents, (especially copies of birth certificates) with the care you would want someone to treat your personal information.
