Departmental Overview: / Health and Aged Services has a workforce of approximately 200 staff involved in service delivery, strategic and service planning, coordination and policy roles. The Health Services team works to promote and maintain high standards of public health and wellbeing, with a focus on disease and illness prevention, with key service responsibilities covering Food Safety, Public Health and Immunisation and an important role in Council’s Emergency Management Plan. Aged and Disability Services undertakes planning, policy, advocacy, partnership and service delivery roles in providing quality home and community support services for older people, people with a disability and their carers and in the development of strategic plans for our older community.
Position Objectives: / To provide holistic living at home assessments, referral and information service to intending or existing clients of Council’s HACC programs and appropriate external services;
Facilitate integrated services and care arrangements via effective liaison with relevant service providers;
Develop and implement care and service plans ensuring that there is a focus on the capacity of each individual to achieve their optimum level of functioning and participation

KEY RESPONSIBILITIES AND DUTIES

Client assessment and review
  • Undertake with older residents and residents with a disability and their carers who are referred to any of Councils programs service specific and/or
holistic living at home assessments for Council
and appropriate external services
  • Adopt a person-centred approach to ensure clients and their carers are assisted to define their own needs and goals and to make informed choices from the range of available Council and external services.
  • Ensure client service access is based upon assessed need and delivered within the criteria of the Priority of Access guidelines
  • Liaise with other service providers – provide appropriate and timely assessment outcomes and/or referrals in order to ensure all needs identified through the assessment process and as agreed to by the client or their carer are actioned.
  • Comply with Occupational Health & Safety requirements, ensuring that any risks or hazards indentified in clients’ homes and service delivery point are noted and subsequently, control measures are implemented.
  • Monitor and review complex and/or short term service clients to identify their ongoing and changing needs, to ensure the range of HACC and external services being received continue to be relevant and deliver effective outcomes.
  • Advise the Coordinator Assessment & Service Planning and/or their nominee on matters relating to client risk and consult in matters of difficult/ complex cases.
  • Work with the Co-ordinator and other team members to monitor the assessment, priority of access and review criteria, to ensure an effective and explicit basis for service access in accordance with program guidelines.
Care planning
  • Create, implement and facilitate appropriate individual care plans that detail service needs relevant to the identified strengths, specific skills, interests and complexity of care requirements of the client and/or their carers in order to promote an integrated and seamless service response.
  • Organise the provision of services outlined in the Care Plan, liaising closely and appropriately with the client/carer/family, HACC Team Leaders, direct care staff and organisations providing the services.
  • Ensure the needs and goals of individual clients and service requirements are clearly identified in care plans and care plans are reviewed and changed in a timely manner.
  • Facilitate and/or participate in case conferences for clients as required and document on SCTT Care Coordination as appropriate.
Intake & Information provision
  • To perform an intake and information role as required for the Service Access Officer during period of leave - to coordinate all referrals into Banyule City Council Aged & Disability Service Unit.
  • Provide information to HACC eligible and non- eligible residents regarding the range of services and activities available including Council Aged & Disability Services, other community care and support options, residential care, health promotion and recreation activities.
  • Participate in the promotion of Council’s Aged and Disability Services to individuals, community groups and other agencies providing related services.
  • Liaise with other Council service units, service delivery and case management agencies in relation to individual clients referrals and services, as required.
Service integration and coordination
  • Establish and maintain effective relationships with local service delivery agencies and consumer advocacy bodies.
  • Actively participate in internal and when required external case discussion and conferences to ensure effective service delivery and coordination.
  • Provide advice and where appropriate advocate service delivery options in relation to individual clients, with HACC Team Leaders.
  • Ensure that short term assessments and services are reviewed and coordinated in a timely manner.
  • Provide timely feedback in relation to the performance of direct care staff, identified at assessment or reassessment are discussed with the appropriate HACC Team Leader.
Administration
  • Maintain client records including the documentation of client consultation to ensure that data is accurate and up to date on the Aged and Disability Services software
  • Ensure the security of all confidential client and service information, taking into account the Privacy Act and Health Records Act.
  • Ensure service provision is planned within financial and other resource limitations.
  • Prepare reports, statistics and participate in questionnaires/surveys as required.
  • Actively participate in assessment team and unit team meetings and planning forums.
Policy and planning
  • Manage client service delivery and service development portfolios, working with other team members with the aim to enhance capacity of service delivery options
  • Assist in policy and procedure development as required by the Coordinator Assessment & Service Planning and/or the Coordinator HACC Service Delivery.
  • Contribute to policy and service reviews and the implementation of service development initiatives.
  • Participate in the implementation of outcomes and initiatives of the Banyule Nillumbik Primary Care Alliance.
  • Provide opportunities for consumer participation and feedback regarding the quality of service delivery, planning and development.

External Liaisons:Clients, their carers, family members, health professionals of community agencies, case managers and personnel from State and Federal Government agencies
Accountability and Extent of Authority: /
  • Under the direction of the Coordinator Assessment & Service Planning and/or nominee within relevant legislation and the policies and procedures of the Council, the officer is responsible for:
  • Assessing and reviewing older persons, persons with a disability and their carers to determine service eligibility and needs to enable access to HACC and external provider services
  • Working directly with service users in establishing their needs, developing & reviewing identified goals and client-centred service options to enhance wellbeing, independence and participation. This includes the planning and organisation of case meetings, referrals and care plans.
  • Supporting direct care staff and rostering officers with complex client issues, in the absence of and/or consultation with Team Leaders
The Officer is accountable for objective and professional decision making about the service eligibility and needs of older persons, persons with a disability and their carers of HACC services and appropriate external providers
Judgment and Decision Making: /
  • Provide authoritative advice to the Aged and Disability Services Team Leaders and Coordinator on issues associated with assessment and intake, to inform and assist with policy development and professional support
  • Provide information for effective decision- making particularly in situations requiring an urgent and immediate response, and utilise discretion to prioritise and plan work outputs to meet with the performance expectation of the position.
  • Required to work with a high degree of autonomy in client management and supervision is generally in the form of pre-determined objectives and guidelines or in the form of special tasks, projects or assignments.

  • The ability to communicate with older people, people with disabilities and their carers, utilising appropriate verbal, written and alternative communication devices as required

  • Demonstrated experience and skills in assessment, care planning and care coordination.

  • An awareness and understanding of the Occupational Health and Safety Act 2004, Equal Employment Opportunity Legislation and guidelines and disability legislation
  • Ability to speak a second language highly regarded

Key Selection Criteria: /
  • Tertiary qualifications in Nursing, Social Work, Allied Health, Disability, or related field (minimum bachelor degree level)
  • Relevant experience in undertaking service specific and comprehensive assessments, intake, and care coordination
  • Demonstrated initial needs identification, care planning and assessment skills in Aged Care and disability services
  • Extensive knowledge and experience of issues surrounding Aged and Disability services, particularly in HACC
  • Knowledge of community services available in the community
  • Ability to collect information in a sensitive manner, with particular regard to cultural requirements, language issues, special communication needs, privacy and confidentiality
  • Ability to manage and prioritise a variety of complex tasks concurrently with minimal supervision
  • Demonstrated communication skills, both written and verbal
  • High level of proficient IT skills – previous experience with aged and disability services software would be an advantage
  • Current Victorian Drivers Licence
  • The successful applicant must be prepared to undergo a police check with ongoing police checks every three years and be prepared to obtain a Working with Children

Banyule Management System: /
  • Conduct all work according to the requirements of the Banyule Management System, incorporating the Banyule Management Manual and Service Unit specific Quality and Cost Standards and procedures.
