CNU Chapter CES Request

Instructions

Complete this form and return it to CNU one month before your event. Complete only the blank fields and leave the Program Number blank - CNU has already filled in some of the required information and will assign you a Program Number. All programs must be real-time. If you have an event with multiple sessions, fill out a form for each individual session.

After you return your request, CNU will send you a sign-in sheet for use at the event to record the names of AIA members who earn credit. You will be responsible for returning the completed sign in form to CNU so we can enter in the individuals.

Chapters are also responsible for sending any certificates of completion requested by attendees. CNU has a template that chapters may use to create certificates. Please direct any questions to the Chapter Coordinator, .
