Each week, a new table from the HIQRP Report is included in the CMO Weekly Update to let you know where we stand in relation to our industry peers.This week, the table shows ARMC’s performance on our Patient Safety Indicators (PSI).As you can see below, Adena’s performance in this time period is comparable to facilities across the nation.

Inpatient Clinical Documentation Integrity (CDI)Reminder

Urosepsisis a non-specific term in the ICD-10 code set. It is not considered synonymous with sepsis. It has no default code in the coding Alphabetic Index, so it will no longer default to a UTI. Should a provider use this term as a diagnosis, he/she must be queried for clarification.

Population Health

The Population Health Team has been working side by side with National Church Residency, since February, actively managing high-risk COPD patients within the ACO. May’s dashboard reveals important information regarding patient need in terms of activities of daily living, social determinants to care, admissions and readmissions. The metrics show: Patient age distribution within this high-risk group is between 75-84 years old; the top two patient needs are, medication management and meal preparation; chronic pain and Hypertension are the two secondary diagnoses associated with this population. Through the Team’s efforts we had good news in May that shows ER visits declined to only one patient for the month of May; and we also hit our COPD target.
