We at Cheyenne Mountain Counseling & Wellness Center are happy to offer you this packet of employment terms. Please take a moment to review this and the rest of the packet in its entirety. The deadline to return this offer agreement is Dec 30th, as we will be taking professional pictures that day.

WHATS NEXT?

If you choose to accept this offer, here is what you will need to do before February 1st, 2014.

(1) Take this free personality test at and this free leadership style test at smalley.cc/images/Personality-Test1.pdf and return your results with your other documents. Be sure to keep a copy for your records for your first staff meeting!

(2) Read the Employee handbook, sign the acknowledgement and return.

(3) Fill out the Direct Deposit form and Return (highly recommended)

(4) Let Admin know what your credentials should read on your card.

(5) Get onto the group scheduling calendar

(6) Get a phone extension and voicemail established

(7) Inform management of retirement choices

(8) Sign the Confidentiality and other policies pertaining to workplace procedures and return.
