Dear Watson Health Care:
I am pleased to inform you that your bid to provide parent aide services for the Lapeer Department of Human Services has been recommended for possible funding. Before an award can be made, several processes within the Department of Human Services must be completed.
Within one (1) week of receipt of this letter, send an email, with the contract number in the subject line, to:Barbra LaRue , Contract Administrator at . Attach the electronic version of the work plan from the bid submitted, with its budget. Please then follow-up with a telephone call to the Contract Administator to provide your agency’s Federal I.D. number (or Social Security number if this contract will be with an individual).
You will be contacted by a representative of DHS shortly to finalize details of the proposed contract. Contract approval steps will be explained in that meeting. DHS is not financially responsible for any services provided in advance of execution of a contract.
