Laboratory RECALL – Beckman Coulter has issued a Class 1 Recall on Synchron LX 20, LX 20 PRO, LXi 725 Clinical Systems. These clinical systems are on agreement PP-LA-255. Beckman Coulter has received reports of erroneous/aberrant sodium, potassium, chloride, and calcium results that have been released from the laboratory. Electrolyte shifts can be low or high and may occur above, below, or within the normal range. There have been reports that patients have been admitted to the hospital and treated based on erroneous test results.

Suggested action steps:

·  Immediately disseminate the attachment to the letter (via notification/alert procedure appropriate to your institution) to all physicians and other healthcare providers served by your laboratory to alert them to the potential for erroneous electrolyte results.

·  Please confirm that you have notified your physicians and healthcare providers via the enclosed response form.

·  Consider the use of an alternative methodology (e.g., backup analyzer, point-of-care device), if available, to analyze sodium, potassium, chloride, and calcium.

·  Periodically, within a laboratory shift, run in parallel some patient samples on the main analyzer and the backup analyzer to confirm system performance and identify discrepancies.

·  Where possible, alternate the main analyzer and the backup analyzer for these analytes to confirm result accuracy.

·  Review patient results using delta checks, where possible.

·  Be aware of abnormal results on sequential patients during an assay run.

·  Run all electrolytes on each sample to enable Anion Gap calculations that will provide additional confidence in the electrolyte results.

·  Be aware of system flags indicating ratio pump errors, critical values and suppressed electrolyte results.

·  Make available osmolarity tests if requested.

·  Please promptly report to Beckman Coulter directly or through your laboratory any aberrant results after receipt of this letter. Please review, complete, and return the attached fax back form.

Nursing - The CRM Collaborative is scheduled to launch on June 2nd. A few key points and reminders:

·  There is no fee to participate, however there is the expectation that members will submit data and actively participate in all conferences.

·  Members that have a non-disclosure agreement in place may still participate in the Collaborative, however they will not receive benchmark information nor will they receive a customized report of opportunities.

·  The data collection tool will be a modified version of the CRM Cost Modeling tool. The intention is to forward the data collection request to registered participants by next week to allow them to begin the data collection process. The data is due back to Premier on June 10th.

·  All activities will take place virtually and no travel is required.

·  Members are strongly encouraged to participate in all conferences. The Collaborative is set up as a series of WebEx’s with each building upon the work and discussion of the previous conference, therefore members are discouraged from a la carte participation.

The CardiacAdvisor website is being updated and will serve as the repository for all CRM Collaborative materials. An email notification will be sent to participating members and the field once the site is complete.
