CLIENT RELEASE

I understand that, despite The Dawg Haus Bed ‘n Biscuit & Beyond, LLC employees’ or agents’ (henceforth referred to as “The Dawg Haus”) best efforts to maintain the safety of every pet and human at the facility, there are certain risks involved in pet care. These risks include, but are not limited to, my pet getting injured during an altercation with another pet and my pet contracting fleas, kennel cough or some other communicable illness. I voluntarily accept these risks, and release The Dawg Haus and its employees, independent contractors, owners and assigns from any and all claims arising out of injury or damage in any way related to or resulting from my association with The Dawg Haus, including but not limited to claims of injuries to my pet, myself or anyone I send to pick up or drop off my pet, or to any property that belongs to me.

I understand and agree that pets are unpredictable animals, and that if my pet becomes injured while at The Dawg Haus I will be responsible for my pet’s behavior, veterinary bills and any other costs incurred due to any incidents caused by my pet. I agree that my credit card, provided in on the Veterinary Care Permission sheet, may be used to pay for any such costs or, if no card information is provided; that I will accept full responsibilityfor damages inflicted by my pet and will pay the costs upon my return. I understand and agree that this release applies to all instances of attendance and to all pets that I own or bring to the facility.

I understand that though The Dawg Hauswill attempt, in an emergency, to contact my pet’s usual veterinarian and myself; such an emergency might not provide the time to do so prior to the administration of care. I therefore grant permission forThe Dawg Haus to attain medical attention for my pet from any qualified veterinarian and to transport my pet to and from that veterinarian when such medical care is deemed important for my pet’s health and safety. I grant The Dawg Haus full power of decision involving the medical treatment of my pet.

I understand and agree that: if my pet damages property belonging to The Dawg Haus I shall be responsible for payment of those damages; and that The Dawg Haus may use my credit card to pay for the damages. I further understand and agree that if my pet attacks and injures another pet (an altercation between pets is one-sided and my pet is at fault), I will be responsible for paying any and all damages caused to that pet, and that The Dawg Haus may use my credit card to pay for those damages.

I represent that my pet is currently in good health and has not had any communicable illness of any kind for one month prior to attending The Dawg Haus. I further represent that each time I bring my pet to The Dawg Haus, I am re-certifying that my pet is in good health and has not had any communicable illness of any kind for one month prior to such attendance.

I represent that my pet is currently protected by the required vaccinations and preventatives and that my pet will be protected throughout each and every day my pet attends The Dawg Haus, each time I bring my pet to The Dawg Haus I will provide certification of vaccinations and preventatives.

I represent that my pet does not have a history of aggressive behavior towards other pets or humans; has not displayed any type of aggressive behavior – recently or in the past; or that I have informed the staff of The Dawg Haus of any such behavior that may have occurred.

I warrant that I am at least eighteen (18) years of age and that I have the full, complete, and unrestricted right and authority to enter into this release.
