  1. Introduction

Please state why this case is important and needs to be reported, including a brief literature review. The background of the case should be described, including the nature of the fungal disorder, its usual presentation and progression, prior treatments and interventions relevant to the case, and a comprehensive but concise description of the presentation if it is a new disease.

  1. Case

This section should present all relevant details concerning the case including: a description of the patient’s relevant demographic information(omitting any information that might jeopardize the patient’s anonymity, such as date of birth, hospital or physician); any pertinent medical history of the patient; the patient’s symptoms; any clinical tests that were conducted and their results; and a description of any treatment given or intervention done. This section may be broken into subsections with appropriate subheadings (‘Differential diagnosis,’ ‘Treatment’, ‘Outcome and Follow Up’ etc.)

  1. Discussion

This section is meant to put the case into the context of similar cases, to explain specific treatment decisions, and to share additional relevant information that is not included in the case presentation.

  1. Acknowledgements

Please acknowledge anyone who contributed towards the study by making substantial contributions to conception, design, acquisition of data, or analysis and interpretation of data, or who was involved in drafting the manuscript or revising it critically for important intellectual content, but who does not meet the criteria for authorship.

  1. Conflict of Interest

Please declare any financial or personal interests that might be potentially viewed to influence the work presented. Interests could include consultancies, honoraria, patent ownership or other. If there are none state ‘there are none.’

References should be in Vancouver style and numbered sequentially in the order in which they first appear in the text and included in the same numerical order in the Reference section. References should be formatted as follows:

Figures and Tables
