Welcome to Clarenbridge Nursing Home

Welcome to Clarenbridge Nursing Home. This booklet has been designed to provide you with the necessary information required to make an informed choice on your care provider and/or assist you in making your stay with us more comfortable. It will also meet the requirements set out in the ‘National Quality Standards for Residential Care Settings for Older People in Ireland’ laid down by the Health Information and Quality Authority. These standards are enacted by the Health Act 2007 (Care and Welfare of Residents in Designated Centres for Older People) Regulations 2013).

This booklet will provide an introduction to the care and services provided within Clarenbridge Nursing Home. Further information and more detailed explanations of the governance and management of the nursing home will be found in the supplementary “Statement of Purpose and Function” booklet.

We Provide

  • Pre admission assessment to determine suitability of placement.
  • Full assessment and consultation on admission to determine care plan and this is reviewed three monthly with resident/family and more frequently if care needs change.
  • Access to GP and pharmacy services.
  • 24 hour nursing care by Registered Nurses and Health Care Assistants.
  • Access to specialist care including Physiotherapy, Occupational Therapy and Specialist Nurses in Wound Care and Neurological conditions if required.
  • Daily choice of food at each meal.
  • There are choices of fresh fruit, cereal and daily snacks on our dining room buffet table and water coolers available for residents and visitors.
  • Laundry service if required. All laundry will have a personalised label.
  • Religious service as appropriate.
  • Use of assistive devices equipment to assist care.
  • Access to specialist services such as when required.
  • Assistance, where required, to aid communication between residents, their families and friends.
  • Transport to and care assistance atappointments which isat an additional fee for the resident.
  • Activities programme which can be tailored to personal choice and includes music, art, dancing, card games and board games.

Rights

Each resident in Clarenbridge Nursing Home has the right to:

  • Receive a contract outlining the rights and obligations of both the nursing home and the resident.
  • Quality care which is appropriate to his or her needs.
  • Participate in the formulation of his or her care plans and to be informed of all services that may be relevant to their needs regardless of their immediate availability.
  • Full information about his or her own state of health and about available treatments.
  • You will maintain control over, and continue to make decisions about, the personal aspects of your daily life, financial affairs and possessions.
  • You shall be consulted on, and choose to have an input into, decisions about your living arrangements in the home.
  • Exercise all civil and natural rights and to have access to services and activities which are generally available in the community.
  • Personal privacy.
  • Live in a safe, secure and homelike environment, and move freely both within and outside the nursing home without undue/unnecessary restrictions.
  • Maintain your personal independence and responsibility for own actions or choices, including those within which there is a degree of personal risk.
  • Take responsibility for your own personal affairs and to undertake daily living tasks of which you are capable.
  • Be treated with dignity and respect.
  • Be accepted as an individual and have preferences for all aspects of healthcare taken into account.
  • Be addressed in a form you are comfortable with in terms of communication ability.
  • Select and maintain family, social and personal relationships with any other person, both within and outside the nursing home.

Responsibilities

Each resident in Clarenbridge Nursing Homehas the responsibility to:

  • Respect the rights and needs of other people in Clarenbridge Nursing Home and to respect the needs of the nursing home community as a whole.
  • Respect the rights of staff and the proprietors to work in an environment which is free from harassment.
  • Care for his or her own health and well-being in so far as he or she is capable.
  • Inform his or her General Practitioner, as far as he or she is able, about his or her medical history and his or her current state of health.

Governance & Staffing

The management and governance of Clarenbridge Nursing Homeis provided by a team of dedicated and committed members of staff who continually strive to provide the highest standards of care. As well as the registered providers, there is also a person-in-charge (Director of Nursing), a deputy Director of Nursing who is the Clinical Nurse Manager II and Clinical Nurse Managers (level 1)

The Director of Nursing is supported by a team of Clinical Nurse mangers, qualified Nurses, Healthcare Assistants, Activities Co-ordinator, Physiotherapist, Occupational Therapists,Housekeeping, Administrative and Maintenance personnel.All external therapists are supervised by Nursing Staff.

Resident Profile

Clarenbridge Nursing Home primarily provides nursing care for both men and women who are over the age of 65. It also provides care to men and women over 18 years of age with specific physical health problems, in particular those with physical and sensory disabilities that are accommodated in ‘Hollylane’. Residents with low, medium and high dependency levels can be accommodated.

As well as long-stay care, we provide short-term convalescent and respite options for both private and public patients.

End of Life Care

Clarenbridge Nursing Home provides end of life care planning which is respectful of the resident’s choices and dignity. This will include physical, emotional, social and spiritual needs as well as the choice of returning home.When required,overnight accommodation and meals will be provided for the family.

In May 2017 the external Boccia court was opened by Michael Donoghue, Galway hurling manager. If you are not familiar with the game of Boccia, we encourage you to give it a try. What makes Boccia ideal for our Nursing Home is it can be adapted to suit all abilities.

Environmental Facilities

Resident Rooms

Clarenbridge Nursing Homeconsists of a combination of bright and spacious single and twin rooms and suites. There are 24single rooms and 22 shared rooms. Clarenbridge Nursing Homeis currently registered for occupancy of 56 residents.

Clarenbridge Nursing Home is in a position to cater for couples that may wish to be accommodated in adjoining rooms or twin rooms, subject to availability.

The majority of resident rooms have en-suite facilities with most of the otherrooms having access to adjacent bathrooms all of which are wheelchair accessible.

Some of our rooms have private gardens, while others have direct courtyard access or meadow views; all of our room sizes exceed those of most standard nursing homes and each resident has their own TV and phone access is provided as requested. Residents can access the daily newspaper by pre-ordering them at the reception desk.Resident’sfamilies can contact residents directly without coming through the main switchboard. If residents wish to make an outgoing call they can do so by contacting reception by dialling “0” or by dialling “9” for an outside line. Internet access can be arranged.

Smoking

Visitors are asked to comply with smoking regulations and not to smoke within the nursing home and to obey the fire regulations. There is a smoking room available for residents that wish to smoke beside the main dining room. It may be necessary for staff to undertake a risk assessment on a Resident to confirm they can smoke independently. There are smoking aprons available for residents who have cognitive impairment and are at risk.

Going out with Family/Representatives

We encourage residents to go out visiting with family and friends, including overnight stays. We ask that this is discussed with staff/management in advance, to ensure the resident has all the medication and other items that they need.

Meal times

Residents at Clarenbridge Nursing Homereceive a balanced diet and necessary nourishment as part of their daily diet.

There is an Executive Chef with a team of staff working within Clarenbridge Nursing Home. They endeavour to provide a variety of appetising, home-cooked and nutritious meals. All meals are made fresh on the day of serving. Menus are designed to meet each resident’s needs. The dining room offers a choice of meals and residents are encouraged to identify their preferred location to dine.

The daily menu is displayed in the dining area to facilitate resident’s choice. Residents are also informed during daily care regarding daily meal choices. In addition, our chef is readily available to discuss any personal tastes/choices and we will do our best to ensure that they are accommodated. We also cater to any special diets that he/she may require and allergens are noted on the noticeboard.

Breakfast is offered from 7o’clock until eleven. Refreshments are offered throughout the day such as (tea, coffee, soup, fresh fruit and fresh scones and homemade cakes).

We operate a full HACCP Quality Assurance standard kitchen and a three weekmenu cycle which includes all the necessary nutrients and nourishments as part of a balance diet for all our residents

Resident Dog

‘Alfie’ Is a cross between a Cocker Spaniel and a Labrador He is of friendly disposition and is very good with residents, staff and children. Alfie is given access to the internal garden when meals are taking place.

Admission Criteria

Admissions to Clarenbridge Nursing Home are arranged by appointment following a pre-admission assessment of your needs. This assessment is conducted to ensure that:

  • the home can meet the person’s assessed needs
  • the home has an appropriate vacancy

Prior to admission, new residents and their relatives will be invited to visit the home to have a look around and to meet the staff. Upon admission residents are allocated to a unit and a named nurse will carry out the admission and document all care needs on the epiccare system.

We understand that in exceptional cases, where there is a vacancy available, emergency admissions may take place. In these instance’s the referral still has to be made via family or professional staff that will provide some brief background on the person to be admitted. This is necessary to allow the nursing home to make a judgement on whether it can meet that persons needs in the short term.

Any emergency admissions are only made as short-term placements in order to allow a further assessment of the persons needs to be made. At the end of this period the resident/ familyand the nursing home will make a decision on whether the person stays or moves on to a more suitable facility. The decision to accept an emergency admission is the responsibility of the Director of Nursing or the deputy – Clinical Nurse Manager II.

First Day

You will be met at the reception area by a member of the Nursing Staff and the Receptionist. You will be escorted to your room and introduced to staff and other residents. A member of staff is available at all times to familiarise you with your new surroundings, ensure that you are comfortable and make you feel welcome. You will have your photo taken for our Epiccare system.

The admission details are recorded by the nurse on epicCare and this assessment forms the basis for the plan of care for each resident. A record of past medical & social history is obtained and this is updated on an on-going basis with current observations on how we can improve life quality.

What to Bring from Home

  • Medical Card number (if applicable).
  • Any medication you arecurrently taking.
  • Clothing for day & nightwear. Ideally 6-9 items of each item. We ask that all items of clothing should be given to the nurse on duty to be labelled discretely so items are not mislaid.
  • Footwear – comfortable non slip laced up shoes and a pair of slippers for night.
  • Personal effects- family photographs, small personal items where possible make his/her new surroundings similar to his/her home.
  • Reading glasses or any other item to make his/her day to day routine familiar to that at home.
  • Toiletries such as toothbrush, toothpaste, deodorants, face cream, brush /comb, shaving cream, razors etc.

Personal belongings shall be entered on an inventory list which will be maintained in the resident record.

Assessment/Care Planning

Upon admission to Clarenbridge Nursing Home asstaff nurse will carry out a comprehensive assessment which will identify care needs; this will be compared with the pre-admission carried out prior to admission to Clarenbridge Nursing Home. From the assessment, care plans will be created with the resident/representative,who shall detail using best practice how care will be delivered.

Resident records shall be reviewed regularly by a nurse to include all the residents’ preferences and best practice. It is designed to maintain independence and ensure personal dignity.

Residents have the right to access his/her resident record at any time and we encourage participation and feedback.

Spiritual Care

Every effort is made to accommodate and support each resident’s preferred spiritual needs and religious observances and requirements.

Representatives from a religious denominationcan be contacted to facilitate religious beliefs and spiritual care.

Mass is celebrated in Clarenbridge Nursing Home on a monthly basis. The resident bus takes Residents to mass in Clarenbridge church every Friday at 10am.

Laundry

It is our policy that each resident has access to freshly laundered bed linen and appropriate clean clothing at all times, a laundry service is provided. All clothing will be labelled discretely with their name.

Clarenbridge Nursing Home employs the services of Pressed for Time external laundry service. Pressed for Time provide an external laundry service between Monday and Friday. This service is provided free of charge to all residents and Clarenbridge Nursing Homewill accept any loss or damage to residents personal clothing.
