DOCUMENTATION & PROCESS FOR THEFT CLAIMS & PHYSICAL DAMAGE CLAIMS THROUGH EDELWEISS INSURANCE BROKERS FOR PURCHASES AFTER 5TH FEBRUARY 2015.

CLAIM INTIMATION:

Claim intimation to be done within 48 hours by logging in to Edelweiss portal through link 203.17.194.17:2070/SangeethaIntimateClaim.aspx by entering the IMEI number and Invoice Number.

For any claims(Theft/Physical damage/Liquid damage)customers MUST & SHOULD upload all the relevant documentsin the portal mentioned abovefor further process.

Or send an Email to: or intimation for all further process (Only if Portal not working).

After intimation and uploading of documents customer has to wait for a verification & approval / confirmation from the Edelweiss team and then send all the original documents by regd post or courier to Edelweiss Mumbai office (address mentioned below).

THEFT CLAIMS: DOCUMENTATION

01.Claim form (with complete information and explain theft in detail, signature, date and place mentioned)

02.Copy of written complaint submitted to Police station

03.Theft DeclarationForm(Letter of Indemnity)

04.Police Acknowledgement stating theft with CSR No.(Complaint should be filed within 24 hours of theft)

05.Sim Blocking Proof(within 24 hours)

06.Discharge Voucher(To be signed by Customer)

07.Specimen Signature ID Proof ( DL/PAN Card/Passport/Attestation of signature by Bank Manager)

08.Original Invoice

09.If Sim is registered other than insured (Father/mother/husband/wife), then customers should submit ID proof of the Sim owner along with relationship establishing document as proof (Marriage Certificate, Ration Card etc).

PHYSICAL DAMAGE:DOCUMENTATION

01.Claim Form (with complete information, details of how damaged is caused, signature, date and place mentioned)

02.Repair Estimate

03.Original Invoice

04.Specimen Signature ID Proof ( DL/PAN Card/Passport/Attestation of signature by Bank Manager)

05.Photograph of damaged handset with IMEI number

06.Cancelled Cheque with name of customer/copy of pass book 1st page/ copy of bank statement

07.Discharge Voucher

After Approval from Insurance Company: customer has to get the handset repaired and courier the original repair billto Edelweiss office for processing reimbursement.

Please do not advise customers to contact National Insurance directly, process has to be only through Edelweiss.

IMP NOTE: ADVICE CUSTOMERS TO COMPLETE THE CLAIM FORMS CLEARLY WITH DETAILED INFORMATION OF THEFT/DAMAGE AS REQUIRED BY INSURANCE COMPANY, ANY INCOMPLETE INFORMATION WILL NOT BE CONSIDERED TO PROCESS CLAIMS.
SIGNATURES ON DISCHARGE VOUCHER: TWO SIGNATURES ARE MUST.
INSURANCE WILL NOT BE PROCESSED IF THE CLAIMANT/CUSTOMER IS BELOW 18 YEARS.
PLEASE ADVISE CUSTOMERS TO USE SIM IN THEIR OWN NAME AS IT AVOIDS DELAY OR REJECTIONS AT THE TIME OF CLAIMINING INSURANCE. THIS IS A MUST AS IT IS A CLEAR RULE OF IRDA.
