City of Waupaca 2016 Wellness Incentive Program (Outline and Instruction Sheet)

Introduction and Purpose- This program is designed to provide an organization wide health and wellness enhancement program, for all employees to enjoy and benefit from. This program is created to motivate participation, by all employees, in various health and wellness related activities that can positively impact their overall health. The City of Waupaca Wellness Committee has created a fun and challenging scorecard program incentivizing and challenging employees to enrich their physical and mental wellbeing.

Employee Participation in the Wellness Program- The wellness program is voluntarily open for all employees to participate. For those participating, points will accrue for successful completion of various activates.

Privacy of the Wellness Information- Any information provided on the wellness tracking sheet will not be shared with any party and only be seen by the wellness Coach. No health information will be requested to violate any federal or state laws such as HIPPA or GINA.

Timeline of the Program- We are kicking off the program March 1st and it finishes on December 31st, 2016. However, if you have any documentable preventative (PRE) events that you attended in January and February, 2016, and provide proof of attendance, you may submit those points along with your March tracker sheet.

Structure of the Wellness Scorecard- Please keep your scorecard easily accessible as a visual reminder to record your points daily/weekly/monthly. There are 5 areas in which you can accumulate points: Preventative (PRE), Education (ED), Give Back (GB), Physical Activity (PA), and Share Your Story (SYS). Explanation and examples of each are listed on the right side of the Monthly Tracking Sheet. Use the calendar on the tracking sheet to help you calculate your points in each area. On the bottom right of the tracking sheet you can add up your points in each category and insert your monthly point total.

Submitting Monthly Sheets- Each month you will add up your points in each area and then the total for the month. Your tracking sheet is then submitted to a department folder or in the Wellness folder at City Hall, by the first Wednesday of the following month. (Example: Submit the March tracking sheet by the first Wednesday in April.) Kristi will be recording the points onto a Master score sheet on a monthly basis. Attach copy of prof for documentable events such as an eye exam. Please do not e-mail your tracking sheets to Kristi!

****All participant tracking sheets, and copies of supporting documents, need to be turned in no later than- Wednesday, January 4th, 2017 for the final point tally. ****

Incentive Program Level of Achievement- there are three point levels to strive for, along with incentive prize drawings that correspond to the levels. Participants that accomplish 75 points are included in the drawing for a Bronze level prize. Participants that accomplish 150 to 299 points are included in the drawing for a Bronze and a Silver level prize. Anyone achieving 300 or more points will have their name in for all three levels.

Date of Drawing- Thursday, January 12th. We will begin with the Gold level drawing first.

*PLEASE NOTE- Maximum of $600 prize value limit per employee.

Classification Prize drawings

Bronze = 75 points per calendar year 6 drawings- $100 gift certificate

Silver = 150 points per calendar year 3 drawings- $100 gift certificate plus Fit Bit

Gold = 300 + points per calendar year 1 drawing- $500 travel voucher
