Dear Applicant,

We are pleased and excited that you are considering planting a church in the Potomac Ministry Network. The Potomac Ministry Network is strongly committed to church planting.

It should be recognized that the Ministry Networkwill act as a facilitator in the process of bringing together a needy community, a group of believers, and a pastor in Biblical relationship.

The office of Church Planting has been created to provide support, encouragement, and accountability to the process of church planting.

The following pages include the steps to plant, an application to be filled out and sent to the Church Planting office along with a resume, references, and authorization and release form. After we have followed through on the references, we will contact you to set up an initial interview.

It is our prayer that God will use this process to further His Kingdom.

AUTHORIZATION AND RELEASE

Having filed an application for planting a church in the Potomac Ministry Network of the Assemblies of God, I consent to have an investigation made as to the conduct of my personal affairs, my moral character, professional reputation and fitness for the church planting ministry and such further information as may be received by or reported to the Potomac Sectional Committee. I agree to give any further information which may be required in reference to my past history.

I authorize and request every person, firm, company corporation, governmental agency, court, association, church, educational facility, or institution having control of any documents, records, and other information pertaining to me, to furnish, orally or in written form, to the Potomac Ministry Network of the Assemblies of God any such information, including documents, records or other information regarding charges or complaints filed against me, formal or informal, pending or closed, and to permit the Potomac Ministry Network of the Assemblies of God or any of its agents or representatives to inspect and make copies of such documents, records, and other information. I shall have the right to inspect or review any information provided to the Potomac Ministry Network of the Assemblies of God, its agents or representatives by any person or organization.

I hereby release, discharge and exonerate the Potomac Ministry Network of the Assemblies of God, its agents and representatives and any person furnishing information from any and all liability of every nature and kind arising out of the furnishing or inspection of such documents, records, and other information or the investigations made by or on behalf of the Potomac Ministry Network of the Assemblies of God. The Potomac Ministry Network of the Assemblies of God shall not be required to verify any information received during the course of its investigations, and shall not be liable for acting on the basis of any information which later appears to have been false or incomplete.

I further agree to indemnify and hold harmless the Potomac Ministry Network of the Assemblies of God and any person, firm, company, corporation, governmental agency, court, association, church, educational facility, or institution furnishing such information, pursuant to this Release, from any and all claims or causes of action, resulting from said disclosure, and against all expenses, including reasonable attorney’s fees, incurred in connection therewith.

I have read and signed the foregoing Authorization and Release as my own free act and deed.
