Child Care Assistance Team

The Child Care Assistance Team is responsible for determining initial and ongoing eligibility for all child care programs. The work involves a high volume of phone and in person interactions with customers and child care providers. The work includes: resolving complex problems using multiple computer systems, authorizing service, and coordinating service.The team works closely with Employment Services Providers and child care providers in the coordination of services. The program helps families pay for child care, so parents can work, look for a job, or attend school and is available for children younger than 13 and for children with special needs under the age of 15.

Shelter Team

The Shelter Team works with precariously housed and homeless families and single adults in Hennepin County. The team develops case plans with families and works closely with internal and community partners. Their primary focus is on reducing the length of stay in shelters and increasing the number of families moving from shelters to permanent housing.

Emergency Assistance Team

The Emergency Assistance Team interviews and processes applications for families who are in a financial crisis that poses a direct threat to the physical health or safety of a child under the age of 18.

Youth Eligibility Team (YET)

The YET team works across Hennepin County to ensure qualifying youth receive and maintain benefits. YET maintains a caseload of both single youth under the age of 20 and pregnant and parenting youth under the age of 20.

Diversionary Work Program (DWP)

The DWP team is responsible for handling cash, SNAP, health care applications, and the review and processing of pending cash cases. DWP performs work that involves extensive client interactions, in person, and on the phone while resolving complex problems using available county resources. The DWP team works closely with Employment Services providers and Daycare Assistance workers in the coordination of services.

Long Term Care / Aging and Disability Services (ADS)

ADS determines and maintains eligibility for all program clients in Long Term Care, group residential housing, and on waivers. ADS performs work that involves customer interactions, in person, and on the phone by resolving complex problems using available county resources. ADS is responsible for the coordination of services by working closely with case managers and facility providers.

North Memorial Team

The North Memorial Team is responsible for resolving complex problems, the coordination of services, and identification of areas for process improvement with clients and financial counseling staff at North Memorial.

HCMC Team

The HCMC Teamis responsible for resolving complex problems, coordination of services, and identification of areas for process improvement with clients and financial counseling staff at HCMC.

Quality Management Team (QMT)

The Quality Management Team (QMT) members have extensive knowledge of assistance programs, systems, and tools used to determine initial and ongoing eligibility. QMT provides information and answers needed to complete quality casework. The QMT assists with corrective action plans, case reviews, training, communications, and serves as liaisons to outside auditors.

Managed Health Care (MHC) Team

The Managed Health Care team is responsible for resolving complex issues with medical services and health plans, providing advocacy for recipients with questions and working to connect recipients with the medical services they need and are eligible for as well as processing enrollment into health plans.
