Checklist for students interested in a Health, Wellness, and Public Safety (HWPS) Coordinated Entry Programs
Step 1. ______Enroll as a CNM student (if you are not already a CNM student). Go to the CNM homepage and click on the “apply” button.
Note: If you have been absent from CNM for three or more terms,you are changing from a high school-aged program, or you last took a non-credit course you must reapply to CNM
Step 2. ______Receive CNM ID number and CNM email address. Check email address to make sure it is functioning properly, and contact Information Technology Services if you have questions.
Step 3. ______If you attended other colleges/universities, have all transcripts sent to the CNM Records Office, and allow time for processing. When evaluation is complete, you will receive an email showing how credits were evaluated.
Step 4. ______Check your record in myCNM to make sure the courses (the minimum required courses) are shown with the EXACT course numbers. If you took a similar course that did not transfer over exactly as the CNM required course number, complete an Appeal for Re-Evaluation of Arts and Science Credits Form and submit the form to the CNM Records Office. Course documentation must accompany this form as stated. If the request is approved, the transfer coursework will appear on your CNM transcript, which can be viewed through myCNM.
Step 5. ______Complete online Pre-Registration Screening form when available (form will be located on the Coordinated Program Entry Office Page) Each program will have its own Pre-Registration Screening form. Make sure to read form instructions carefully. Students will not be eligible to enroll in program courses without completing a Pre-Registration Screening form.
Step 6. ______Check your CNM email address regarding your Pre-Registration Screening Form. Students who clear the pre-registration screening will be given information on program registration date/time. It is the student’s responsibility to check their CNM account regularly.
Step 7. ______Register for program courses on given date/time. Seats are limited, enrollment in program courses is not guaranteed
