Checklist and declaration - Instructions

The following checklist and declaration is to be completed by the coordinator on behalf of the consortium and should be signed and added when sending the Final Technical Implementation Report.

This checklist should help consortia to submit a complete Final Technical Implementation Report. Incomplete reports / missing documents and information lead to delays in the final payment and/or cuts in the eligible cost.

The declaration serves as confirmation that the consortium has acknowledged the authors when existing material was used and that the project did not receive funding from other EU programmes for the performed work.

Checklist for Final Technical Implementation Reports (FR)

Check before submission
The final technical implementation report is written in English.
The final technical implementation report follows the guidelines provided on the IEE website.
Hard copiesof the deliverables, in number and languages, were agreed with the project officer.
The report includes a Publishable Report and a Confidential Management Report.
The final technical implementation report has been proof-read and approved by the co-beneficiaries (with exception of individual performance reviews if any).
The final technical implementation report informs about the implementation of the whole project.
The instructions for the use of the IEE logo, disclaimer and reference to IEE funding have been followed, especially for publication of the publishable report, key deliverables and the project website.
The publishable summary slides and the project factsheet are fully updated. The texts are of publishable quality without further editing.
All deliverables are in their final version.
The Confidential Management Report includes:
an updated list of submitted deliverables of the action,
the table of hours spent by each partner during the action,
asigned declaration regarding the use of existing materials(see below),
the list of contact persons after the end of the action.
Submission
POSTAGE: The consolidated package to be sent within 60 days (at the latest) after the end of the action to the EACI address as indicated in the grant agreement includes:
  1. a cover letter indicating the grant agreement number, acronym and title of the action, in which the coordinator requests the payment of the balance,
  2. two hard copies of the final technical implementation report (publishable report and confidential management report) with all appendices (FR),
  3. a CD-ROM, DVD or USB stick with the electronic version of the technical and financial reports and all deliverables,
  4. hard copies of key deliverables as agreed with the project officer,
  5. one original and one copy of the signed final financial statements with all appendices (FFS). For details on the completeness of the FFS, refer to the admissibility of financial statements (see also ).

E-MAIL: An email with the final technical implementation report (without deliverables) and the final financial statements (with appendices) to be sent to .
WEBSITE
The project website is fully updated (indication that the project is completed; no use of future tense; no 'expected' outcomes; no outdated 'news'; etc.).
All key deliverables with public dissemination status (PU) are available on the project website.
The project website will be kept accessible until at least 2 years after the end of the action, in line with the IEE guidance for project websites. The beneficiary responsible for maintenance of the project website has been indicated in the consortium management chapter of the final technical implementation report.

Declaration

I, ______(project coordinator), confirmthat, whereexisting material was used in deliverables of the IEE project [ACRONYM, project number], it has been properly referenced.I also declare that no part of the work performed under this grant agreement has been funded by another EU programme.

Furthermore I confirm having received assurance from all my co-beneficiaries that they have complied with the same rules.
