Allen Foundation’s Champion ExcellenceGrant program solicitsprofessional development opportunityrequests that further the mission, vision, and the FOCUS values of UnityPoint Health. All opportunities must be compatible with IMPACT Allen Hospital’s purposes to:
- Enhance the patient experience
- Improve the atmosphere for Allen Hospital team members, physicians, or volunteers
- Provide higher level of training and education that fosters engagement and unity
The Champion Excellence Grant program is intended to provide funds for professional development, otherwise not supported through budget, such as meals, lodging, registration costs, travel expenses etc. Funding will not include education credits that are reimbursed by Allen. Please refer to the Allen Health System policy “Education Reimbursement” for clarification.
Who is eligible to apply:
- Team members, excluding physicians and providers, of Allen Hospital, College, Hospice, Black Hawk Grundy Mental Health, and UnityPoint Health- Waterloo clinics not associated with an affiliate hospital. (Providers have appropriate CME funding available to them on an annual basis).
Champion Excellence Grantprogram guidelines:
- Maximum grant request is $10,000 per professional development opportunity.
- Only one application per person/ per opportunity may be submitted.
- Applications require Manager approval and Supervisor signatures.
- Before submitting a request, please check with The Allen Foundation to see if your department has funds for expenditure. All grants that are awarded will be reimbursed to the individual after the professional development opportunity has taken place. All opportunities must take place after the grant application is awarded.
Champion Excellence Grant administrative guidelines:
- Complete all sections of the application which is available at in The Allen Foundation Office.
- Submit8stapled copies of the application (and budget quotes as needed) to the Allen Foundation. Please limit all supporting documents to three pages.
- The Allen Foundation will notify successful applicants the week of December 3, 2018.
