Certified DOT Physical
Dr. Lon Edwards is licensed by the State of California (13506) and is nationally registered with FMSCA (9789299734)with 20 years experiences to perform DOT physical exams and issue Certified Medical Examiner Certificates. The charge for the examination is $100.00 and includes a urinalysis (blood, sugar and protein) and a Certified Medical Examiner’s Certificate. The urinalysis test is not a drug test.For all complex medical examinations (multiple health problems) the fee ranges from $125.00 to $200.00.

What to expect at your visit:
1)Including the DOT Medical Examination Report with “Driver Information” and “Healthy History” section completed.{Page 1 will be filled out by driver}(Doctor will go over this section.)
2)You will be examined according to the FMCSA regulations that are listed on the bottom of this page but you may expect: Vision, traffic signal colors, blood pressure, pulse, eye movements, ears, peripheral vision, neck, lymph nodes, spine for tenderness, spasm, range of motion, abdomen for tenderness, hearing, reflexes, balance, neurological checks and a hernia check (pain in the abdomen or testicles).(Doctor will do this section)
3)While we make every effort to accommodate walk-ins, those with appointments will be given a priority. We have all of the necessary funds that are required; you need only bring a valid driver’s license and be prepared for the urine test.
4)We’ll complete your Medical Examination Report for Commercial Driver Fitness Determination form, give you a summary of your exam and provide a Medical Examiner’s Certificate as appropriate.(Green Card)
5)Insurance is not accepted for this service. We cannot prescribe or refill prescriptions.
6)While many people tell us this is one of the best physicals they have had, it is NOT intended to substitute for routine health evaluations by your primary care provider. (We do not do cholesterol, prostate or colon exams.)
I certify that the information was read and all questions about the Certified DOT Exam were answered. You need to make a copy of the exam for your personal record. All federal privacy acts will be follow (HIPAA).
