Cat Rescue Network will: (To be retained by adopter)

● Provide the adopter with the animal described as follows: ______

·  Ensure that the animal has been checked by a veterinarian for signs of:

- Infectious conditions - Skin irritations i.e., rash/missing fur - Parasitic infestation

- Severe depression or behavioural problems.

Animals with serious medical conditions will not knowingly be placed for adoption.

● Ensure that mature animals have been spayed /neutered and undertake to have kittens sterilized by Cat Rescue Network's vet once they reach about 6 months of age.

Ensure that an animal over 16 weeks of age has had its initial vaccinations unless contraindicated. Kittens will be vaccinated at 12 to 20 weeks old if robust, and spayed/neutered at about 6 months of age at Cat Rescue Network’s expense. The adopter will be responsible for the boosters;

● Ensure that the adopter is informed of any minor medical conditions Cat Rescue Network is aware of and provide the adopter with any medication the animal is currently receiving (eye ointment, antibiotics etc) but will not be responsible for any ongoing treatment or medication once the animal has been placed with the adopter.

Cat Rescue Network will not be responsible for any veterinary fees incurred without their written permission. If the animal requires medical care up to 10 days following the adoption, the animal can be returned to Cat Rescue Network for a full refund. Cat Rescue Network cannot undertake to pay the veterinary fees of owned animals.

Spay/Neuter Arrangement:

Cat Rescue Network will retain ownership of the animal until it has been sterilized, at which time the ownership transfers to the pending- adopter.Cats will be sterilized prior to adoption unless there is a medical reason for delaying the procedure. Kittens will be sterilized at 5 or 6 months of age.

The pending-adopter must contact Cat Rescue Network coordinator, , (613) 820-7088 before the animal is to be spayed/ neutered and the coordinator will authorize coverage of the procedure with a CRN veterinarian; and send a copy of that authorization to the pending-adopter. The pending-adopter can then book the appointment. A reminder will be sent when the animal is due to be sterilized. Veterinary cost, other than vaccinations or spay neuter, are the responsibility of the pending adopter.

If the pending-adopter prefers to have Cat Rescue Network's animal altered by a veterinarian other than one of Cat Rescue Network's veterinarians, the Cat Rescue Network is willing to accommodate this request but the adopter will be responsible for covering the additional sterilization cost and proof of sterilization must be provided.

If the cost to spay/neuter the cat increases significantly between the time of the pending adoption and the time the animal has the procedure done the pending adopter will be responsible for the difference. CRN will notify the pending adopter of the increase and will authorize the procedure once the additional cost has been covered. It is important to have the procedure done in a timely manner to avoid additional costs.

I am adopting the cat/kitten for myself and will not sell or give the cat/kitten away to anyone else. If the cat/kitten can no longer be kept, I will give Cat Rescue Network the first option to take it back. If CRN cannot take the animal back CRN will offer to assist in finding it a home.

I will:

● Contribute towards Cat Rescue Network’s cost of caring for the animals at the rates listed below. Cheques are to be made payable to Cat Rescue Network.

● Understand that Cat Rescue Network cannot hold a cat unless the adoption fee has been collected. The adoption fee is not refundable if I change my mind and CRN has held the cat. Once the screening has been done, if CRN determines that it is not a suitable home for that cat, CRN will return the adoption fee in full.

● Ensure that the animal receives a booster vaccine. If the cat has received its booster, the adopter will reimburse Cat Rescue Network the cost of obtaining the booster.

● Not declaw or otherwise mutilate the cat/kitten, and will return the cat/kitten to Cat Rescue Network rather than declaw it.

● Allow Cat Rescue Network to do a pre and post adoption visit if requested or otherwise contact you to follow up on the cat/kittens health.

● Allow Cat Rescue Network to check with their veterinarian for references prior to adoption.

● Ensure that the animal is sterilized on or close to the due date If I delay having the animal altered and there is a surcharge by the veterinarian for an animal in oestrus (heat) or pregnant, the surcharge is my responsibility;

● Have the cat/kitten seen by a veterinarian regularly to ensure its continued good health. I will seek veterinary care if the cat appears to be ill or is behaving abnormally.

● Keep the cat/kitten in a clean, comfortable, safe environment. Provide for daily exercise and access to a window. Provide my cat/kitten with proper cat/kitten food, along with constant fresh water daily. Groom the cat/kitten regularly. "Cat/kitten -proof" the home. Properly supervise the cats/kittens while around children and strangers for the safety of the animal and people.

● Notify Cat Rescue Network if I can no longer keep the cat/kitten; and agree that the cat/kitten shall NOT be sold, given to, or adopted to anyone else, nor shall there be any transfer of ownership unless Cat Rescue Network has been given the option to reclaim the cat/kitten or screen the new home if for any reason I can no longer keep it.;

● Agree that Cat Rescue Network retains superior title to said cat/kitten: limited to and for the express purpose of assuring the well- being of the cat/kitten. This includes NOT declawing the cat. Cat Rescue Network will only exercise its superior claim in the event that it appears to Cat Rescue Network that the proper and humane care as specified above is not being afforded to the cat/kitten.

● Agree to abide by all local animal ordinances and anti cruelty laws.

It is understood that: (To be retained by adopter)

● Cat Rescue Network cannot guarantee the temperament of the cat but will disclose the information they know about its temperament, behaviour, health and background. Therefore, in consideration of receiving the cat/kitten, the adopter hereby releases Cat Rescue Network from any and all claims of liability.

● Cat Rescue Network makes every effort to place only healthy cats/kittens but cannot guarantee the extended health of any cat/kitten adopted from them, or be responsible for any post adoption medical expenses that may be incurred. Cat Rescue Network limits its warranty to a ten (10) day refund or exchange policy. Any other expenses, including medical treatment, if needed, are the responsibility of the adoptive parent(s).

● The adoption fee is to help Cat Rescue Network defray the expenses for this cat/kitten and other cat/kittens, and is not refundable after the ten (10) day grace period. The adoption fee includes spaying or neutering, first vaccination and any surgeries or other medical treatments that were needed while the cat/kitten was under the care of Cat Rescue Network.

● If the cat is returned at any time within the ten (10) day grace period CRN will refund all but $50 of the adoption fee. CRN will try to take the animal back but will not refund the adoption fee after the 10 day grace period.
