Job Description for LD Senior Care Worker

The main purpose of the job is to ensure that people who use Fremantle’s services receive high quality care and support, including Support Planning and administering medication, in accordance with their assessed needs.

Line Managed by either Service/Unit Manager

Key Responsibilities

  • Provide respectful high-quality support and care for people who use Fremantle services in accordance with their individual Support Plans.
  • As key worker for individual people who use services, develop with their participation a person-centred Support Plan ensuring that their assessed needs and wishes are comprehensively met. Ensure the Plan is kept up to date, obtaining appropriate input from relatives, other professionals and staff colleagues.
  • Practically assist people who use services with tasks of daily living such as personal care and mobility, and end of life care, as directed in the Support Plan.
  • Support people who use services to take their medication as prescribed and recorded in the individual Medication Administration Record.
  • Ensure that people who use services have companionship and social, emotional, leisure, occupational and educational support in accordance with their wishes and the Support Plan
  • Ensure that people who use services receive planned or urgent health care whenever required.
  • Record essential information in people who use services’ daily reports, care and health monitoring charts, accident and incident reports, and service message systems clearly, accurately and immediately.
  • Ensure that day-to-day tasks and routines are carried out to contribute to the smooth running of the service as a whole.
  • Actively promote the service and Fremantle by clear, respectful and helpful communication with all potential customers including people who use services, relatives, health and social care professionals, other officials and contractors, visitors as well as Fremantle colleagues.
  • Contribute to the success of the whole staff team by respecting colleagues and participating in team meetings and communications systems.
  • Provide supervision (known as ‘key sessions’) and induction support for staff colleagues with the guidance of the line manager.
  • Taking responsibility for specific training as directed by the Service Manager.
  • Any other duties consistent with the business of Fremantle, thejob, and hours of work as may reasonably be required.

Each staff member must also take joint responsibility with their line manager for their own personal and professional development at work in accordance with the Performance Development and Support Policy.

Attainments, Experience & Qualifications /
  • Good numeracy and literacy skills
  • Understanding of the principles of care practice and acceptable risk taking
  • Experience of writing and implementing Support Plans
  • Specialist knowledge of client group
  • Minimum 1 year experience working in a health or care service setting
  • Understanding of the principles of equality, diversity and anti-discriminatory practice
  • Understanding of relevant health and safety legislation
  • Able to participate in household tasks

  • NVQ LevelIII in Care (or higher) or equivalent
