1INTRODUCTION

A new pathway has been proposed which allows many patients to be followed up by selected trained and accredited community optometrists at approximately 4 weeks.

At the end of cataract surgery, patients appropriate for follow-up will be identified and referred to a chosen optometrist for follow-up at 4 weeks. Those not suitable will continue along the current UHNS pathway

The Cataract Follow-Up system uses the Ework environment

Ework is a Process Management tool that is particularly suited to replace paper based processes where several people may interact.

A process is a flow of information and activities.

Users can:

A Follow-Upsare created at the UHNS clinic. A staff member at the clinic opens a new blank form, completes patient details, operations detail selects an accredited optometrist

B The selected optometry practice receives an email notification. The practice acknowledges receipt of the Follow-Up within 2 working days.

C Post appointment, examination results are added by the practice. An email notification is sent to the GP practice and UHNS.

3.2Live system

Within the North Staffs network the live system can be access from favourites, click favourites on your browser then NHS Links / IT Services / Ework. This will log you in automatically using your network account but requires that the optometry roles have been added to your account specifically, otherwise use the following link

3.3Log In

Open your web browser and enter the URL provided into the Address bar. Call up the URL and a screen similar to that shown in Figure 3 will be displayed. Click on the service name to log in.

The following Log In window is displayed.

Enter the username and password above and then click the green arrow.

4THE EWORK ENVIRONMENT

Users login to Ework using a Web browser, a window similar to the followingappears:

4.1Components

The components of the Ework environment include

List Selection Buttons –

To Do – This button displays all Process folders from all processes that are currentlyon your To Do list.

Watch List – This button displays all Process folders from all processes that arecurrently on your Watch list.

Blank Forms – This button displays forms from published processes. Only forms towhich you have been granted access are displayed.

Administration Forms – This button displays administration forms from publishedprocesses. Only administration forms to which you have been granted access aredisplayed.

Clicking a List Selection button for the currently displayed list refreshes data displayed inthe list.

Logout Button – This button allows you to log out

List Filter – This filter allows you to specify the items that appear on the currently selectedlist.

Paging Buttons – these allow you to navigate the pages of the current list.

Filtering and navigation of lists is described in the section on Process Lists.

List – this is the list viewing area

4.2Notes on ework Forms:

Fields with a red edge are mandatory and must be completed before submission

Grey fields are read-only - ie for information only.

Each Follow-Up has a unique reference labelled 'FolderRef' displayed on each page.

After 6 months, the forms are archived. The archive can be searched using the Cataract Follow-Up Search administration form

5OVERVIEW OF THE UHNS ROLE

UHNS clinic staff can

5.1Start a Follow-Up

A Follow-Up is started when a member of staff fills out an electronic form in the same way as they might a paper form. The form is electronically sent to a selected optometrist practice. The practice is alerted by email when a new form has been created.

5.2Track a Follow-Up

UHNS staff can track the progress of their forms online. They can identify the stage it has reached and can see activities associated with it.

6USING THE SYSTEM

6.1How to Start a Follow-Up

Log on to eworks

Click on the Blank Forms button

Click on the 'Cataracts' process in the grid

Complete the form

Patient Details

Enter patient details directly or search for a patient using some search criteria

Add UHNS number if known

Add a Daytime phone number

Operation Details

Operation Date and eye fields are mandatory. Any relevant comments may be added in the Notes field.

One or more electronic files may be added by clicking on the + on the Add Files box

and navigating to the file. Files may be deleted by selecting the file then clicking on X

Optometrist Details

The optometrist list displays practices with at least one accredited cataract optometrist. The list is initially ordered alphabetically by name but can be ordered by nearest practice to the displayed postcode.

Click on a practice in the grid. Detailed practice information is displayed (if known) including unavailable dates

It is mandatory to select an optometrist from the grid - a system check alerts users if they have not selected a practice.

Click on the green Submit arrow. A message informs the user that the follow-up has been sent.

6.2How to Track a Follow-Up

Log on to eworks

Click on the Watch List button

A list of Follow-Ups appears in the grid.

The list may be filtered by selecting a stage in the 'Filter Tree' on the left. Click on 'Cataracts' in the filter tree to display all stages.

The list may be ordered by clicking on the green title bars

The progress of a form is indicated by the stage name

Stages:

New Cataract Follow-Up (form has been sent to optometrist but not yet acknowledged)

Wait for Cataract Appointment (form has been acknowledged, appointment date may have been displayed)

Cataract Test Completed (indicates client has attended appointment and results added)

Click once on a Follow-Up from the grid to view (Read-Only) / Print the original form or test Results.

Forms may be printed in a PDF format

Notes may be added at any stage by clicking on the 'Add Notes' link at the bottom of a form. Notes may be viewed by selecting the 'Notes' tab at the top of the page.
