CANCELATION AND NO SHOW POLICY
We understand that situations arise in which you must cancel your appointment. In order to be respectful of the needs of all our patients, please be courteous and call if you are unable to show up for your appointment. Please call 970-565-7195, even if it is after hours with at least24hours prior to your scheduled appointment to cancel. If you leave a message and want to be rescheduled, please leave a number where we can return your call. Cancellation notices under the 24 hour notice are considered no shows and are subject to a NO SHOW FEE.
A "No Show" or a Late Cancellation is someone who misses an appointment without cancelling prior to 24 hours’ notice of their appointment or a failure to be present at the time of a scheduled appointment. A no show will be recorded in your medical record.
- First missed appointment: there will be no charge
- Second missed appointment in 12 months: $25 fee will be collected per appointment to reschedule (this fee will be held on your account as a credit and will be applied if you attend the scheduled appointment. If you do not show to scheduled appointment, fees will be retained by the office)
- Third missed appointment in 12 months: $50 fee collected per appointment to reschedule and applied as above with the possibility of being discharged from our practice
- Cancellation or no show fee questions can be directed to the office manager
FAMILY APPOINTMENTS
Often times we will see several family members on the same day. Cancellations of entire families create a huge burden to the staff and doctor. It is imperative that if you need to cancel the entire family that you call 24 hours in advance. On the same note, if you wish to exchange one family member for another, please call in advance of this change so that we can prepare the record and authorize insurance. We may not be able to accommodate this change if we are not made aware until the time of appointment. Cancelation of a family may require us to break the family up to reschedule future appointments and/or reschedule fees as defined above. Ideally we recommend that you schedule no more than 2 family members a day unless an approval from the office manager is granted. If you know you will be late with your family appointment, please call us at the earliest opportunity. We will do everything we can to see each family member that day, however if you are more than 10 minutes late, we may have to reschedule one family member for another day and will be marked as a No Show in patient record.
LATE APPOINTMENTS
Your scheduled time is your appointed time, any time after this time is considered a late appointment. We will do everything possible to see you the same day but it may require you wait for us to work you in or it may require that we reschedule you. If you know you are running late please call at your earliest opportunity, this will allow us to manipulate the schedule to accommodate you. However, we may need to reschedule you based on how late you will arrive.
