General Guidelines
Inclusion/Exclusion Criteria
Introduction
TN Disability Pathfinder, UT Boling Center, Memphis Public Library & Information Center, YWCA of Greater Memphis, Memphis Center for Independent Living and Latino Memphis havepartnered together and created a reliable resource database that providesdisability services, mental health services, and social services for the Latino community inShelbyCounty. All the agencies included in this database have bilingual staff.
Guidelines for agency inclusion/exclusion criteria are needed to provide direction on the type ofdisability services and support programs to be included in the bilingual database. This programcontains some comprehensive resource information along with the disability resources.
Disclaimer and Referral Guidelines:
Inclusion in the Camino Seguro West Bilingual Resource Database should not be considered asan endorsement, rating, recommendation or evaluation of any given service or provider.Likewise, no negative evaluation or rating of a program or service should be inferred due toexclusion from the database.While this database will strive to give the most accurate and appropriate information and referralpossible, agency information does change. Each agency in the database will be updated atleast once every six months.The partner agencies are not responsible for the quality of service delivered by any agencylisted in the database.
Agencies/Programs Eligible for Inclusion:
Agencies that have at least one person on staff that is bilingual (can be part-time)
Nonprofit agencies that provide unique social services to the Hispanic community
For profit agencies/programs that provide services needed that are not otherwise
available, such as automobile/van adaptations or emergency alert systems, animalaides, adaptive driver training, translation/interpretation
Governmental (public) agencies that provide human services (financial assistance, food,transportation), disability services, education, housing, low-income health clinics,* andlow-income dental services
Community and advocacy organizations available to the disability and Spanish-speaking community
Public school systems and vocational rehabilitation programs
Churches that offer community-based services that have been in existence for at leastsix months and have at least one full-time staff person that runs the community program
Agencies/Programs NOT Eligible for Inclusion:
Private, for-profit service providers unless they meet one or more of the specific
inclusion criteria listed above
Services that are available only to members of a specific organization or affiliation
Churches that offer no special community-based service components for the generalpublic
Individual, for-profit child care facilities
Any organization that promotes or delivers services that are illegal
Any organization that declines service on the basis of color, race, religion, ethnicity,nationality, sexual orientation, or ability
IMPORTANT: These policies do not prohibit the inclusion of any programs that target servicesbased on age, gender, health, disability or other characteristics designed to meet the specialneeds of the targeted populations.
Guidelines for Complaints/Removal of Agencies/Programs:
In general, agencies may be removed if they meet one or more of the following criteria:
Failure to adhere to Inclusion criteria and guidelines
Documented history of poor customer service
No longer provide bilingual services to individuals
Data Elements:
The following data elements will be used for the purpose of gathering resource information to beincluded in the database.
Organization name (legal name) and AKAs
Legal status (e.g., nonprofit, government, for-profit)
Hispanic contact
Additional bilingual staff
Location and mailing address
Contact telephone number
Eligibility requirements
Services provided and target populations served
Documents that may be required by the organization for application (such as birth
certificates, etc.), eligibility requirements, intake procedures and application process isoptional
