Greek Organization Prospective New Member Form
Each new member must sign and return a copy of this form to the Greek Life Council before participating in any chapter or prospective new member activity.
Academics
I give my consent to the Greek Life Council to release my educational records to my chapter for use in determining my eligibility for membership in the chapter as well as scholarship programs, advising, and chapter averaging at any time during my membership in the chapter. I authorize this release for the time that I am a student at Minot State University, or until I notify, in writing, the otherwise.
Membership
I understand that, as an obligation of membership,
- I shall not use race, religion, handicap, age, or national origin as criteria for voting or influencing other’s votes in any matter in my organization. I understand that I may refrain from voting if my comments shall be so biased.
- I may be required to live in the chapter house if eligible.
- I will be required to fulfill financial obligations in a timely fashion to both Minot State University and my chapter. I understand that the chapter may charge members the cost of repair to the organization and that these charges may be prorated among all chapter members.
- My actions, whether independent of the group or as a member of it, reflect upon the chapter and of Minot State University.
- I understand that should I wish to terminate my membership in the organization that I must notify chapter officers and the Greek Life Council promptly and file a resignation form. I understand that it is necessary for members to have accounts paid before termination of membership, in accordance with current regulations and Minot State University rules. I understand that resignation through the Minot State University alone does not in all cases release me from my obligations to the affiliated organization in which I am a part.
- I acknowledge that the Student Welfare and University Affairs Committee and/or the chapter of which I am a member will be charged with the responsibility to determine when individual actions are detrimental to the community as a whole, and if an action is deemed detrimental, the Student Welfare and University Affairs Committee will apply appropriate sanctions.
- I agree to abide by all of my chapter’s (and affiliate chapter, where applicable) rules and regulations, constitution, bylaws, policies and procedures, as well as Minot State University policies, and local, state, and federal laws.

Anti-Hazing Policy
As per the Student Conduct Policy, found within the Student Handbook, Minot State University offers the following statement on hazing:
“Hazing by university groups is prohibited on or off campus. Hazing is defined by North Dakota Century Code 12.1-17-10. A person is guilty of an offense when, in the course of another person’s initiation into or affiliation with any organization, the person willfully engages in conduct that creates a substantial risk of physical injury to that person or a third person. As used in this section, “conduct” means any treatment or forced physical activity that is likely to adversely affect the physical health or safety of that other person or a third person, or which subjects that other person or a third person to extreme mental stress, and many included extended deprivation of sleep or rest or extended isolation, whipping, beating, branding, forced calisthenics, overexposure to the weather, and forced consumption of any food, liquor, beverage, drug, or other substance. The offense is a class A misdemeanor if the actor’s conduct causes physical injury, otherwise the offense is a class B misdemeanor.”
I understand that I have the obligation to ensure that my chapter upholds the principles upon which it was founded as well as to protect my own dignity. I acknowledge the position of Minot State University, the Greek governing councils and the national headquarters of my organization regarding hazing: Hazing in any form should not be tolerated. I acknowledge that I have read and understand the Minot State University Anti-Hazing Policy. I further acknowledge that I am subject to all federal, state, Minot State University and national headquarters rules and policies. It is my responsibility to report violations of these policies and procedures to the proper authorities.
