Directions: Fill out the application. Acquire a copy of your transcripts or grade report and attach it to the application. Applications may be turned in to Mrs. Cocksedge in N-18 or you may put in her box in the attendance office.

  1. Fill out the chart below with your course and grade information from the previous semester.

Use the lists on the reverse side to determine where your courses should be listed. (If you do not see your course listed, you may not include it.)

CSF Points are granted as follows

  1. A = 3 Points (AP Courses = 4)
  2. B = 1 Point (AP Courses = 2) *No more than 2 AP points allowed per semester
  3. C = 0 Points
  4. A grade of D or F in any course, even one you cannot use to qualify, disqualifies you from membership at this time.

You must have a minimum of 10 Points from last semester’s grades to qualify:

  1. The first 4 Points must be from List I
  2. The first 7 Points must be from Lists I and II
  3. The remaining points may come from any List, I, II, or III.

You may use no more than 5 courses to qualify.

No CSF points are given for PE courses, courses taken in lieu of PE, courses repeated to improve a grade, courses involving clerking and office/teaching assisting, and courses taken on a pass/fail basis. Consult with your advisor regarding Summer School courses.
