DEFINITIONS

  1. BOCES: a board of cooperative educational services established pursuant to Article 5 of Title 22, C.R.S. for the cooperative management and service delivery to multiple school districts.
  1. Care Coordination Plan: a plan developed by the school district using the Care Coordination Plan guidelines from the Department, and submitted through CDE. The plan describes how the school district will achieve service integration, monitoring, and advocacy with other service providers to assist clients in gaining access to needed medical, social, educational, and other services.
C.CDE: the Colorado Department of Education.

D.Department: the Department of Health Care Policy and Financing.

E.Disability: as it pertains to school–based targeted case management,a physical or mental impairment that substantially limits one or more major life activities.

F.Individualized Education Program (IEP): an individualized plan, developed in accordance with 105-17, Reauthorization of Part C-H of IDEA that formalizes the goals, objectives and specific special education and related services needed by a student in special education.

G.Individualized Family Service Plan (IFSP): an individualized plan, developed in accordance with 105-17, Reauthorization of Part C-H of IDEA for a child 0 - 3 years of age who has a developmental delay or condition that may cause developmental delay, which identifies the early intervention services needed by the child and family.

H.Individual Health Services Plan (IHSP): an individualized treatment plan developed in accordance with the Procedure Guidelines for Healthcare of Students with Special Needs in the School Setting, published by the Department of Health and the Department of Education. An IHSP is developed for a student who has a serious or chronic health problem for which services are required during the school day so that the student can function in the school setting. These services would not or could not normally be provided by the student’s primary health care provider.

EXHIBIT A continued:

I.Interperiodic EPSDT Screens: EPSDT screening and diagnostic activities that are medically necessary to determine the existence of suspected physical or mental illnesses or conditions. The necessity for these screens is determined by a health, developmental or educational professional who comes into contact with the child outside the formal health care system.

J.Local Services Plan: a document developed by a school district that chooses to enter into a contract with the Department to claim reimbursement for school health and related services. The plan, developed in coordination with CDE and written according to guidelines issued from the Department, describes community input, the types of health services provided, how a needs assessment has been done, the types of health services needed, and how match money will be spent.

K.Medically at risk: as it pertains to school-based targeted case management, a recipient who has a diagnosable physical or mental condition having a high probability of impairing cognitive, emotional, neurological, social or physical development.

LPartial EPSDT Screen: a component of the comprehensive EPSDT screen performed outside the established EPSDT periodicity schedule to identify or assess a suspected illness or medical condition and the need for further diagnostic or treatment services. A qualified health care professional operating within the scope of his/her practice shall perform the partial screen.

M.Provider: according to 26-4-103, C.R.S., as amended, any person, public or private institution, agency, or business concern providing medical care, services, or goods authorized under this article and holding, where applicable, a current valid license or certificate to provide such services or to dispense such goods and enrolled under the state medical assistance program.

N.Qualified Health Professional: an individual who meets State or national requirements for licensure, registration or certification as a health care professional who operates within the scope of his/her practice.

O.Section 504 Accommodation Plan: the individualized plan, developed in accordance with the Federal “Rehabilitation Act of 1973”, 29 U.S.C. sec. 701, for a student who has a physical or mental disability which substantially limits one or more major life activities (such as breathing, walking, self-care, etc.), but who is in a regular education setting or not eligible for special education. The plan identifies the specific services required by the student while at school.

P.School District: a public school district, a board of cooperative educational services, or a State educational institution that serves students in kindergarten through twelfth grade, organized under the laws of Colorado, except a junior college district.
